Intrahepatic cholestasis of pregnancy is a gestation period unique disease which have the characteristics of skin itches and jaundice.It belongs to high danger pregnancy,At present,the treatment of ICP mainly is Ursodeoxycholic acid,Cholestyramine, Phenobarbital andDexamethasone et al.The clinical symptoms will occur again if we stop using the medicine.These medicines usually have side effects if we use them for Long-term.Traditional Chinese medicine has specially adjustmental function for ICP,and it has no obscure side effects.Therefore,Yinchenhao was used to research its function and possible actional mechanism for ICP from blood biochemistry value,pathology change of liver,the expression of ICAM-1 on liver.Part1 The establishment of animal model for intrahepatic cholestasis of pregnancy ratsObjective:To provide basis of biochemistrial and pathologial experiment for successful establishment of the rat model by exploring the changes of the blood biochemistry value and hepatic pathohistology in pregnant rats with Estrogen and Progestron induced intrahepatic cholestasis. Methods:The animal model of intrahepatic cholestasis of pregnancy rats was induced by estradioli benzoas and Progesterone.All of rats were divided into normal group,model group,high dose group,middle dose group,and low dose group of Yinchenhao and ursodeoxycholic acid group at random.On the 21st day,the blood of all pregnancy rats was collected by picking the eyeballs.After all rats were put to death with luxation on cervical vertebra,two clumps of liver tissue of each rat in normal group and model group were made into light microscope and electron microscope specimens routinely.The serum level of alanine aminotransferase,aspartate aminotransferase, total bilirubin,total bile acid of rats in normal group and model group were measured after the blood was centrifuged. Results:The level of every biochemistry value was decreased remarkably in model group than that of normal group(P<0.01).Pathology change of liver:under the light microscope,the cell structure of liver in normal group was clear,and there was no denaturetion and necrosis.In model group,hepatocellular structure disorder,hepatocellular necroses,vacuole-like changes and hepatic sinusoid very narrow in hepatic partial region were observed.Under the electron microscope,bile capillary in normal group did not expand.There was no bile salt in bile capillary.In model group,bile canaliculi were seen dilated.And the morphological changes in dilated bile canaliculi were found as follows: decrease or disappearance of superficial microvilli,deposition of substances with high electronic density,dilatation of smooth endoplasmic reticulum,tumefaction of the mitochondria and loss of their ridges.Conclusion:The pathological changes showed intrahepatic cholestasis inpregnanct rats.The changes of the blood biochemistry value and hepatic pathohistology in pregnant rats demonstrated it was successful to establish an animal model of intrahepatic cholestasis in pregnanct rats.Part 2 Effect of Yinchenhao on blood biochemistry value of pregnancy ratsObjective:To provide basis of biochemistrial and pathologial experiment by exploring the effects of Yinchenhao on the blood biochemistry value of pregnancy rats.Methods:The level of ALT,AST,TBIL,and TBA were tested by biochemistry analyzer.Results:The level of ALT,AST,TBIL in each medical group had obvious difference with model group(P<0.05 or P<0.01),high dose group and UDCA group had the best effect,and they had no difference with normal group(P>0.05).In the improvement of TBA,UDCA group had the best effect,it had no difference with high dose group and normal group,and the level of TBA in each medical group had obvious difference with modle group(P<0.05 or P<0.01).Conclusion:Yinchenhao might be actived by improving the blood biochemistry value of pregnancy rats so as to attain the goal of treating ICP.Part 3 Influence of Yinchenhao on the ICAM-1 of liver in rats with intrahepatic cholestasis during pregnancyObjective:To research the expression of ICAM-1 on liver so as to explore the possible adjustmental effect and the action mechanism of Yinchenhao up on ICP.Methods:The expression of ICAM-1 on liver were detected by immunohistochemistry stainging. Results:The expression of ICAM-1 on liver in model group had obvious difference with normal group(P(0.01).hfter treatment,the expression of ICAN-1 on liver in each medical group had obvious differfence with model group(P<0.05 or P(0.01),high dose group had the best effect.Conclusion:Yinchenhao could inhibit the level of IChM-1 on liver which indicate the possible action mechanism up on ICP.
