Objective1. To work out a feasibility and practical home-basic care programme for patients with chronic diseases on the base of the home-basic care demand state of chronic diseases patients .2. To explore the effect of the home-basic care programme on blood pressure ,blood glucose , mental health and quality of life (QOL) in essential hypertension and type 2 diabetes mellitus(T2DM) patients.Methods1. First of all, 8 out of 16 imperative and feasible home-basic care items are selected according to the home-basic care demand state of chronic diseases patients, then combining with related reference,working out home-basic care programme with patients and their dependents together.2. A nonrandom paired study was carried out in Fuzhou ,140 patients with essential hypertension and type 2 diabetes mellitus were invited ;taking sex ,age , pathogenetic condition and type of diseases as paired condition, and were divided into control group and intervention group, the control group were given community usual care, cure group were token home-basic care according to the programme for 6 months,and the intervention groupfor cure group includes grading-up disease knowledge, advancing metal health, strengthening social supports and taking routine care. The intervention groupeffects were evaluated after 3,6 months. The evaluated indexs are consisted of physiological index, psychological index and the scores of QOL ; physiological index include blood pressure , blood glucose and glycosylated hemoglobin .psychological index is comprised the score of Self-Rating Anxiety Scale(SAS) and The Geriatric Depression Scale(GDS). Results1. A home-basic care programme for chronic diseases was worked out , it include five flow chart of nursing intervention ,a home-basic care programme table and a great number of regulations in home-basic care for chronic diseases.2. Application of the home-basic care programme in the patients with diabetes(1) In term of blood glucose ,there was marked differences (P≤0.05)between two groups both at 3 months and 6 months ,the blood glucose of the intervention group is lower than the control group,the better effects at 6 months.(2) There wasn't marked difference in the scores of GDS between the groups at 3 months but that of SAS was .the differences on the scores of SAS and GDS between two groups both had significance(P≤0.05)at 6 months , SAS and GDS scores of the intervention groupare lower than the control group , the better effects at 6 months.(3) On the whole ,the QOL of home-base group is better than that of the control group .At 3 months and 6 months ,there were marked differences in the scores of part items between two group such as role-physical( RP), bodilly pain( BP), general health (GH), role-emotion (RE) and mental health (MH) ; but physical function (PF), vitality (VT) and society function (SF) have no differences;the effects after 6 months is better.3. Application of the home-basic care programme in the patients with hypertension(1) At 3 months and 6 months ,The differences in the effect of decreasing blood pressure between two groups both had significance(P≤0.05),the blood pressrure of the intervention groupis lower than the control group ,the better effects at 6 months.(2) The scores of SAS between two groups has no marked differences at 3 months,but that of GDS has;the differences on the scores of GDS and SAS between groups both have marked significance at 6 months.(3) On the whole ,the QOL of home-basic care group is better than that of the control group .At 3 months ,There were marked differences in the scores of a part of items between two group such as RE, SF,but PF , RP, BP, GH, VT, MH. At 6 months ,the differences between the two groups in RE, PF, GH,MH the scores of have significance(P≤0.05).but there weren't statistically significant on PF, RP, VT and BP.Conclusion1. The home-basic care proposal for chronic diseases is operability and practicality .On the one hand,, it includes concrete, feasible and clear home-basic care programme for chronic diseases ,and flow chart of nursing intervention and a great number of regulations about home-basic care for chronic diseases. On the other hand ,the home-basic care service items are emergency request for chronic diseases and suitable for family.2. The application of the proposal is benefial to control blood pressure and blood glucose, improve mental health and rise the QOL of the patients with essential hypertension and type 2 diabetes mellitus. So it is deserving to be applied in community care.
