Experimental Study On The Mechanism Of Jia Wei Er Zhi Jian In Treating Constipation Predominant Irritable Bowel Syndrome

Objective: To explore mechanism of Jia wei er zhi jian in treating rat model of constipation-predominant irritable bowel syndrome .The aim is to provide experimental evidence for clinical research.Method: 72 healthy Wistar rats were selected and randomly divided into tow groups: normal group, model group. The normal group had 24 rats, they were obversed as control group; the model group had 48 rats, they were intubed with icy water for 4 week.The granule number and the moisture content of the feces from the two groups were measured daily, the body weight was observed each four days. Four weeks later, selected 16 rats from each group randomly, the rectal balloon dilation were performed to evaluate the perception thresholds by the abdominal with drawal refles scores of the two groups, charcoal suspension lavaged, 16 rats were killed to observe following index after 20min: small intestine charcoal suspension pushing rate, estrogen, testosterone, pathology of bowel mucosa tissue.There were 52 rats after four weeks, including 16 normol rats and 40 molde rats of C-IBS. The 40 molde rats of C-IBS were divided into three groups randomly: the blank conrol group, the jia wei er zhi jian group and the cisapride group, the cisapride group had 8 rats, the other groups had 16 rats. The amount of dried medicinal herb given to jia wei er zhi jian group was 17.5g/kg weight; the amount of Cisapride given to cisapride group was 30mg/kg weight; the normal saline was given to blank conrol group; the normal group was gives nothing. The granule number and the moisture content of the feces from the four groups were measured daily, the body weight was observed each four days.Two weeks later, randomly selected 16 rats from the jia wei er zhi jian group, the normal group and the blank conrol group respectively. The rectal balloon dilation was performed to evaluate the perception thresholds by the abdominal with drawal refles scores of the three groups. When we finished the scores, selected 8 rats from the three groups, killed the 16 rats to observe following index: estrogen, testosterone and pathology of bowel mucosa tissue. The colon samples from ileocecal junction and distal colon were stained by toluidine blue for mast cells and by immunohistochemistry for 5-HT immunoreactive cells.At the same time, we selected 8 rats from the jia wei er zhi jian group, the normal group, the cisapride group and the blank conrol group respectively, and raised nothing except water for 24h.Then killed the rats to observe index: small intestine charcoal suspension pushing rate, pathology of bowel mucosa tissue. Meanwhile, we made use of immunohistochemistry for c-kit staining to observe colon samples from ileocecal junction and small intestine.Result:(1)The granule number and moisture content in mordle group were significantly less than those in normol group after the icy water intubed into the rat stomachs four weeks later(P<0.05). Compared with the normal group,the model group is less in the small intestine charcoal suspension pushing rate and the viscera perception threshold to the content .The scores of each grade of AWR, the female rats were lower than those in the male rats(P<0.05).(2)After two weeks in threating with jia wei er zhi jian, compared with the blank conrol group, the counts of MC and the expression of 5-HT positive cells were significantly lower in the ileocecal junction and colon of jia wei er zhi jian group, (P<0.05),and the expression of c-kit positive ICC was significantly decreased (P<0.05). The granule number and moisture content was no siginificant diffrence between jia wei er zhi jian group and normol group,there was no siginificant diffrence too in the small intestine charcoal suspension pushing rate and the viscera perception threshold to the content between jia wei er zhi jian group and normol group(P>0.05).(3) After two weeks in threating with cisapride, compared with the normol group, the granule number and the expression of c-kit positive ICC were decreased. There were no siginificant diffrence between tow group (P>0.05). The time of small intestine charcoal suspension pushing was shorter than blank conrol group.But the moisture content of cisapride group was significantly lower than normol group(P<0.05).(4)The female rats of modle group were haiher than male rats in the contents of estrogen in blood plasm and the viscera perception threshold to the content. There were siginificant diffrence between tow group (P<0.05). But the contents of estrogen in blood plasm were lower and the viscera perception threshold to the content was recover after two weeks by treating with jia wei er zhi jian.(5) The bowels mucosa tissue of rats was not found apparente inflammatory pathological changes。Conclusion: The mechanism of Jia wei er zhi jian in the treatment of C-IBS was to adjust the disordered secretion of MC, 5-HT, ICC, E2 in the bowels mucosa tissue and blood plasm.
