| Objective To analyse the diagnostic value and significance of tumor necrosis factor-α(TNF-α) and tumor necrosis factor receptorⅡ(TNFRⅡ) in premature rupture of membranes(PROM) and investigate the relationship of TNF-α,TNFRⅡand PROM.Methods The studied population consisted of patients from the obstetric of the First Hospital of Shanxi Medical University from September 2008 to December 2008. Subjects were divided into two groups:①36 cases of PROM in pregnant women for experimental group.②40 cases of the normal full-term births in the same period of pregnant women as control group.Diagnostic criteria of PROM is in accordance with "Obstetrics and Gynecology"(sixth edition) of Le Jie editor.Average gestational age are 37~41+6 weeks,single births,head position,no prenatal history of trauma and sexual intercourse,and excepting those as pathological pregnancy,pregnanting combined with medical and surgical diseases,PROM suspicious persons.The same criteria was used in control group.All subjects were balanced in age and motherhood times.Mining centrifugal blood to refrigerator preservation after the mother admitted to hospital.The TNF-α,sTNFRⅡlevels in the serum samples were examed by enzyme-linked immunosorbent assay(ELISA);A piece of membrane(2cmx2cm) near break place was immediately collected after labor,saline washed, formalin-fixed for 24-48 hours with the conventional 10%solution, paraffin-embedded,sliced,stained.The expression of TNF-αand TNFRⅡon fetal membranes was detected by immunohistochemical method.Results①The serum TNF-α,sTNFRⅡlevels of experimental group are significantly higher than the levels of control group.The difference is significant (P<0.05);②48 hours or more time after rupture of membrane,maternal serum TNF-αand sTNFRⅡare significantly higher than that less than 48 hours.The level are higher with longer time after the rupture of membrane;③The membrane cells with TNF-αand TNFRⅡ-positive expression in experimental group markedly increased than control group,the difference has statistical significance(P<0.05).TNF-αpositive granules mainly expressed in the chorionic membrane of the cytoplasm of cells,TNFRⅡpositive granules mainly expressed in the chorionic membrane of the cell membrane and some cytoplasm.Conclusions TNF-αand TNFRⅡcould be the indicators in early diagnosis and estimate prognosis of PROM.The exact mechanism need to be studied to predict further the occurrence,and promote clinical prevention and control work,which will eventually improve the security of maternal and child. |
