Clinical Studies On Using Moxibution As A Complementary Therapy In Moderate To Severe Endometriosis (EMs) After Conservative Surgery

Speciality:OBG of integrated traditional and western medicineAuthor:Yuan Hong-xiaTutor:Professor Feng HongBackgroudThe incidence of Endometriosis(EMs) has showed an upward trend.Due to its complicated etiopathogenisis and pathogenesis,with the limitations in operation and medication,the therapy becomes increasingly intractable currently.As one of the main therapies in EMs,surgical methods such as removaling focus of infection and anatomical reconstruction that do good in releasing pain and improving post-operative pregnancy rate,however,the relapse rate of post-operative patients is high,particularly for those whom choose conservative methods.Therefore,research in treatment for post-operative patients with EMs has been exploring for a long time.Bases upon the source of traditional Chinese Medicine,to explore a new treatment for the treatment in post-operative patients with EMs featuring with simple,easy, effective,workable,and leading to the self-blance which would make sense.ObjectiveTo study the clinical effect and the change of laboratorial index while using moxibution as a complementary:therapy in moderate to severe endometriosis after conservative surgery.MethodA random and compare sampling method was used in this study,and a total of 90 cases with moderate and severe endometriosis after conservative surgery were recruited into the study,which have been divided into 3 groups of moxibustion group,Gestrinone group and combination group.Subjects in moxibustion group were treated with moxibution 3 days after surgery,there are 12 accupoints(CV4,CV6,EX-CA1(dual),BL32(dual),SP10(dual),SP8(dual), SP6(dual) were selected for treating,Qod,4 weeks for a course,andlasted for 6 courses in continuance.Subjects in Gestrinone group were medicated with Gestrinone capsule a week after surgery,2.5mg,p.o,Q3d,and lasted for 6 months in continuance.Subjects in combination group were treated with both two treatments as mentioned above.Subjects' signs and symptoms were recorded before and after treatment.Liver and renal function,CA125 and blood-fat level were also examined at the same time.Result1.The total effective rates of moxibustion group,gestrinone group and combination group were 68%,92.6%and 96.1%,the relapse rates of the three groups were 32%,7.4%and 3.9%.By x~2 test,the difference has statistical significance(P＜0.05).The relapse rate of moxibustion group is Significantly higher than that of gestrinone group and combination group.The difference of relapse rate between gestrinone group and combination group was not significant(P＞0.05).By rank sum test,the clinical effect of combination group is the best.2.The total effective rates with TCM Syndromes of three groups were 88%, 62.9%and 92.3%,the difference has statistical significance(P＜0.05).Moxibustion group and combination group had a lower score than gestrinone group in the sign criterion of traditional Chinese medicine.3.Moxibustion group and combination group' s improvement on TCM symptoms score and VRs score isbetter than gestrinone group(P＜0.05),but Percentile difference between imoxibustion group and combination group was not significant(P＞0.05).4.CA125 level after treatment of the three groups were significantly lower than that before treatment(P＜0.05).CA125 level after treatment of Moxibustion group were significantly higher than the other two groups(P＜0.05).5.TC and LDL-C level of gestrinone group were increased afte Medication for 6 months,the differences between before and after treatment were statistically significant(P＜0.05).Both moxibustion group and combination group had no significant change on lipid indexes.6.side-effects such as weighting of gestrinone group are more than moxibustion group and combination group(P＜0.05).Conclusion To compare the effect of Moxibustion and Gestrinone in the treatment for post-operative patients with moderate to severe endometriosis,Moxibustion presents outstanding results in improving TCM symptoms,alleviating clinical symptoms(dysmenorrhea,non-menstrual lower abdominal pain etc.),but bustion inferiors to Gestrinone in the aspects of clinical efficacy and control at recurrence.Thus,to combine both the two treatments(moxibustion and Gestrinone) in clinical use can definitely improve Clinical efficacy and TCM symptoms,reduce side-effects such as weighting and abnormal lipid metabolism,and help the patients obtain a better life quality.
