[Objective]To investigate the relationship between the serum uric acid(SUA) and target organ damage,ambulatory blood pressure and nondipping blood pressure.[Methods]Choosed the 350 patients with primary hypertension,drawn the vein blood in early morning with empty stomach.The serum level of uric acid was measured by the colorimetric method(Roche/Hitachi Modular Analytics,Roche Diagnostics,Mannheim, Germany).Serum concentrations of total cholesterol,HDL-C,LDL-C,triglyceride and plasma glucose were measured by the direct quantitative colorimetric method(Human Gesellchaft f(u|¨)r Biochemica and Diagnostica,Germany).At the same time,the height, weight of all patients' were measured and BMI was calculated.Ambulatory 24-hour BP monitoring was performed on each patient's nondominant arm by means of the OSCAR 2 oscillometric 24-hour ambulatory BP monitor(MeiGaoYi, Medical,Beijing,China).The device was set to obtain BP readings at 30-minute intervals during the day(06:00 AM-22:00 PM) and at 60-minute intervals during the night(22:00 PM-06:00 AM).Mean blood pressure≥140/90 mmHg(6:00 AM-22:00 PM) and mean blood pressure≥120/80 mmHg(22:00 PM-06:00 AM) was diagnosed hypertension.All subjects were instructed to continue their usual activities during the inspected time and to write usual activity and sleep and wake time.All readings were analyzed by computer.The correct recordings with total recordings exceeding 80%of the readings were been selected. Data were edited by omitting all readings of diastolic BP readings<40 mmHg,systolic BP readings>220 mmHg.[Results]1.The SUA and TG was positive correlation,the SUA and TC was negative correlation in all patients;The relation between the SUA and Cr was positive correlation, The relation between the SUA and age,Glu was negative correlation in male patients;The relation between the SUA and BMI,TG,Cr is positive correlation,The relation between the SUA and TC is negative correlation in female patients;after adjusting multiple cardiovascular risk factors,such as serum uric acid,age,BMI,et al.The SUA in female was an independent risk factor with the LVH,the odds ratio(OR) is 1.159(P =0.046),the SUA in male was an indirect risk factor with the LVH,and the same time the SUA in all patients was an indirect risk factor with coronary heart disease and cerebrovascular disease and chronic renal failure.2.The male patients'SUA,24hMDBP,DMDBP,NMDBP,24hMSBP-load,24hMDBPload,DMSBPload,DMDBPload,NMDBPload,24hMAP,DMAP,NMAP were greater than the female patients,there was significant difference between the two groups(P<0.05),the relation between the SUA and MASBP DMSBP NMSBP is positive correlation.3.Uric acid levels of nondippers' was no significant difference composed with UA levels of dippers'.UA was positive correlation with nocturnal mean beats.The results of multivariate analysis showed that TG(OR=1.210;95%CI,0.977-1.498;P =0.0812) together with age(OR=1.030;95%CI,1.011-1.049;P =0.0016) were associated with the nondipping pattern.[Conclusions]The SUA in female is an independent risk factor with the LVH,the SUA in male is an indirect and not an independent risk factor with the LVH,and the same time the SUA in all patients is an indirect and not an independent risk factor with coronary heart disease and cerebrovascular disease and chronic renal failure.2.The relation between the SUA and SBP is correlation,the SBP of the old primary hypertension patients increases with the SUA increasing and their PP is hightened with the SUA increasing.The SUA could have been acted as prognostic factor with primary hypertension.3.This study is initially to demonstrate that increased serum uric acid levels are indirectly associated with nondipping blood pressure patterns in patients with essential hypertension.
