varicocele is caused by the blood reflux of the pampiniform plexus.The reason of the reflux is the failure of reflow or the damage of venous valve.These make the vein dilatation,bent,prolate.Some researches indicate that varicocele is related to sterility.In primary sterility patients,the incidence of varicocele is 35%(19-41%),compared to 80%in secondary sterility patients.Although varicocele is related to sterility,the mechanism of how varicocele result to sterility is unknow up to now.There are several hypothesis which explain the pathological change and influence of masculine procreative resulted from varicocele.The main hypothesises are:high temperature of scrotum;back flow of metabolism product of adrenal gland and kidney;insufficiency of function of hypothalamus -pituitary gland axis(inhibition of hebin);testis oxygen deficiency and PH change.Generally,The temperature in the testis is 0.6-0.8℃higher than normal.Metabolite refiow means:in varicocele patients,vena spermatica interna valve is hypoplasia or defect.So,left renal vein blood refiow to left vena spermatica interna.Thus,the metabolite of adrenal gland and kidney such as catecholamines,prostaglandin,hydrocortisone,toxic metabolite,5-HT eflow into testis;influence testis tissure;kill spermatozoa and influence spermatic motoricity remarkably.Heat shock protein is a group of protein and its name originrd from heat shock response.In 1962,Ritossa made the temperature of rearing fruit fly from 25 to 35℃.After 30min,he discovered that in a particular region of the salivary gland chromosomes puff occured,prompting that the genetic transcription in the region was enhanced.Ritossa called the phenomenon heat shock response.In 1974,Tissieres' studies showed that the reason for the formation of puff mentioned above was that the heat shock induced gene transcription,translation of specific protein called heat shock protein.After that,similar circumstances were found in stress condition such as ischemia,anoxia,injury, metabolism toxicant et al.So heat shock protein is also called stress protein.Morimoto divided the main HSPs into 4 family-HSP90,HSP70,HSP60,and small HSP.HSPs reside in prokaryotic and eukaryotic cells.They are a group of highly conservative protein family generated in a variety of tress conditions.According to the relative molecular mass and the amino acid homology HSPs can be divided into several subgroups.HSP70 is one of the most extensively and penetratingly studied.In physiological conditions,a small amount of HSP 70 can be created.It participate in a variety of physiological functions such as:enhance the anti-stress increase capability of cell;participate in the composition and role of steroid hormone receptors;perform as molecular chaperone towards the new peptide;elaborate and presenta tumor antigen;maintain cell homeostasis;regulate growth,development,differentiation and death of cell.In 1995,Boulangerd et,al,found HSP70 protein on the surface of masculine germ cell.In vitro,this protein can prevent preven sperm-egg recognition. Matwee,bohring prove that HSP70 involved in the pathogenesis of male infertility in different ways.By measuring the seminal plasma HSP70 levels of infertility patients with varicocele,infertility patients without varicocele and normal human,this experiment is tempted to understand HSP70 expression in the seminal plasma in varicocele patients and to explore its relationship with male infertility.This research selects 25 male sterility patients with varicocele,25 male sterility patients without varicosity stochastically.All the subjects must have the following condition:be health;be sterile but have normal sex life after marriage for more than 2 years;does not have trauma as well as the hereditary H disease history;doesnot have sexual disturbance history;testis,epididymis and spermatic duct are not obvilously abnormal in medical examination;the ultrasound confirms that the varicole is above midrange.The spouse gynecological examination discovery has not been abnormal.Selects 20 healthy males as control group who have children already.Use HSP70 ELISA reagent box to measure the HSP70 content in seminal plasma.Result display:the HSP70 content in varicole male sterility group is 8.54±1.43ng/mL:in male sterility without varicole group the content is 5.13±1.89 ng/mL:The control group is 3.46±2.24 ng/mL.HSP70 content in male sterility group with or without varicole is higher than the control group obviously and the difference has obvious statistics significance(P<0.05).In sterility patient,the HSP70 level in male sterility with varicole group is higher than male sterility without varicole group obviously and the difference has the obvious statistics significance(P <0.05).This research demonstrated:HSP70 content in Sterile group is higher than the control group obviously.This illustrate that HSP70 is one of the reasons which cause male sterility.HSP70 content in varicole male sterility group is higher than male sterility without varicole group and HSP70 leveal in male sterility with varicole group is reduced after High ligation of spermaticvein.It illustrates that varicole is one of the reasons which cause HSP70 elevate in seminal plasma.Therefore,HSP70 in seminal plasma plays an inportant role in pathogenesis of varicole resulting in male sterility.
