IntroductionThe situation of birth defects is grim in China, and prevention is the key to reducing birth defects. The past prevent birth defects system was seriously flawed and there is an urgent need to establish a Community-depended preconception care model based on the condition of our country.Material and methodTo establish the preconception care model, many theories and methods ware used, such as the project management theory, the Structure-Process-Outcome theory, the health technology assessment theory, interviewing experts, summarizing literature and so on.To assessment of intervention effects, we selected Jingan Area of Shanghai as the pilot area. The subjects carried out a survey of three parts: evaluation of community promotion, evaluation of education for married couples and assessment of the satisfaction of preconception screening and counseling.ResultsThe model of community-depended preconception health care including 3 parts: 1) health promotion for resident by the family planning network 2) education for all reproductive age women and her partners about reproductive plan and preconception health, and 3 ) Preconception screening and counseling for free.Evaluation of intervention is based on the Structure-Process-Outcome theory. It was found that the community-depended preconception care model was effective. In the assessment of outcome, 2056 valid resident questionnaires, 2083 valid married couple questionnaires and 532 valid satisfaction questionnaires were collected. The assessments of outcome showed that the health promotion raised the awareness of preconception knowledge of resident, increased the rate of correct contraceptive use (71.1 to 79.6%) and increased the preconception screening rate (37.8% to 52.0 %).ConclusionThe community-dependent preconception health care model relied on family planning network is a model with China's characteristics and China's national conditions.
