ObjectivesNipple discharge is one of the three most common symptoms of breast diseases, the rate of occurrence of which is between 3%-8%.Since both benign and malignant breast diseases may with the symptom of nipple discharge,the clinical diagnosis determine further treatments.Nowadays,the wide clinical uses of fiberoptic ductoscopy(FDS) make it possible for us to observe the pathological changes of large and middle ducts and the opening part of small ducts directly by an amplificatory view,which provide particular references to the doctors with qualities, numbers and locations of the diseases pre-operation.After all,in clinical practices, the pathological finding rate pro-operation was often much lower than the clinical finding rate by FDS pre-operation.At the same time,considering the important function of breasts' location,how to decrease the harm of operation to meet the needs of better appearance and minimally invasive as well as how to upgrade the pathological finding rate became an important clinical lesson.In this investigation, we discussed the practical value and advantages of orientation using FDS during operation by comparing with the traditional operation.MethodsWe divided 60 outpatients cases with nipple discharge into observation group and controlling group randomly.In the former group,we chose minimally invasive operation by locating the neoplasm with FDS.On the other hand,the patients of the controlling group received the traditional operations.Then,the effects including operating time,incision length,specimen size and pathological finding rate were evaluated.ResultsWe found that the average incision length of observation group was 2.67±0.24cm while that of the controlling group was 3.47±0.26cm.Average time for operation was 48.00±7.14min of observation group while 33.00±7.60min of controlling group.Average weight of specimen was 7.73±6.02g of observation group while 43.40±5.89g of controlling group.Pathological diagnosis of observation group included 24 cases of intraductal papilloma,4 cases of ductal epithelial papillary hyperplasia,1 case of mammary duct expansion and 1 case of mastopathy,the pathological finding rate is 93.3%.By contrast,pathological diagnosis of controlling group included 18 cases of intraductal papilloma,2 cases of ductal epithelial papillary hyperplasia,2 case of mammary duct expansion and 8 case of mastopathy,the pathological finding rate is 66.7%.ConclusionsComparing with traditional operations,minimally invasive operations under the assistance of FDS had the advantages of small incision,less damage of glands,and higher rate of pathologic finding,which had great clinical significances in treating breast-duct diseases.
