| Objective:In the combination diseases with syndrome, through collecting 245 cases of chronic gastritis ,to establish the quantizing diagnostic criteria and the degree-classify criterion of Spleen-Stomach Damp-Heat syndrome by symptoms and physical signs .To explore the relevance between the Spleen-Stomach Damp-Heat Syndrome and the micro-target such as Helicobacter pylori( Hp )and gastrin ,so as to lay the foundation for the deeper research of the essence of spleen-stomach damp-heat syndrome .Methods:1. Choose 245 cases diagnosed by endoscopy in chronic gastritis patients, Divide into two groups A and B.each group is divided into two types :non-spleen-stomach Damp-heat and spleen-stomach damp-heat . to collect clinical material, and then fill in the clinical questionnaire of spleen-stomach damp-heat syndrome in chronic gastritis. Reference to diagnostic criteria (clinical study of Chinese medicine guiding principles. 205 cases of Group A for the establishment of quantitative criteria of Spleen-Stomach Damp-Heat Syndrome which is retrospectively examined .40 cases of Group B to quantify the diagnostic criteria for the forward-looking test.2. For target examination:â‘ The Helicobacter pylori examines: Using 14C- urea expiration experiment.â‘¡The blood serum gastrin (GAS) examines: Using of RIA analysis. By observing with positive rate of hp and gastrin to explore the two examinations of the reference value in t diagnosis of spleen -stomach damp-heat syndrome.3. Data statistics: Utilize the multivariate statistical analysis to determine relevant factors; use the conditional probability conversion method to establish factor score table; use the means of discrimination method of maximum likelihood to identify field order of quantitative diagnosis , build the criterion of quantification diagnosis and study its sensitivity , specificity, authenticity,and then quantify the diagnostic criteria for testing and evaluation. All statistics were carried out using SPSS 13.0, tabulation software used excel 2003.Results:â‘ Retrospective study: through the statistical analysis, we established the classing and quantizing diagnostic criteria of Spleen-Stomach Damp-Heat Syndrome in Chronic Gastritis as follows: abdominal swelling and pain (3 points); yellow and greasy coating of the tongue(17 points); thirsty drink less (6 points); bitter taste in the mouth (5 points); loose stool with indigested grains(3 points); nausea and vomiting(4 points); heavy sensation in the limbs and body(5 points); anorexia(2 points); red tongue body(4 points). If a Chronic Gastritis patient's spleen-stomach damp-heat syndrome involved the syndromes above, and his numerical value add up to 24-31, he can be diagnosed as slight spleen-stomach damp-heat syndrome,;if he got 32-40 ,he can be diagnosed as midrange spleen-stomach damp-heat syndrome; if his numerical value larger than 40 ,he can be diagnosed as serious spleen-stomach damp-heat syndrome. The retrospective study shown the results of test about the classing and quantizing diagnostic criteria: sensitivity 91.18%; specifieity 89.32%; accuracy rating 90.24%; masculine likelihood ratio 8.54; Negative likelihood ratio9.69%. â‘¡Perspective study: The perspective study shown the results of test about the elassing and quantizing diagnostic ceriteria: sensitivity 94.74%: specifieity 95.24%; accuracy rating 95%: masculine likelihood ratio 19.89; Negative likelihood ratio4.52%.â‘¢Detect micro-target: the positive rate of Hp infection and serum gastrin in spleen-stomach damp-heat syndrome group were significantly higher than that of non- spleen-stomach Damp-heat syndrome group. (P <0.01)Conclusion:Through retrospective and Perspective study, confirmed the quantizing diagnostic criteria of the sensitivity, specificity, accuracy and positive likelihood ratio were quite high, while the negative likelihood ratio was low. therefore, this is a good diagnosis criteria, has certain clinical usability.â‘¡Hp infection are highly relevant to the Spleen and Stomach Damp-Heat Syndrome in chronic gastritis. elevated serum gastrin may be a "Spleen and Stomach Damp-Heat" micro evidence. |
