The Study On The Regularity Of Diagnosis And Treatment Of Diarrhea In "Treatise On Febrile Diseases"

Objective:There are more than 80 lines referring to diarrhea in "Treatise on Febrile Diseases",including six meridian syndrome,which is widely used in clinical departments.The curative effect of the prescriptions of treating diarrhea in "Treatise on Febrile Diseases" is obvious,which have profound influence to later ages.Our study attempts to classify and summarize the lines involving diarrheain "Treatise on Febrile Diseases" from the aspects of the etiology,the pathogenesis,the therapy and the formula and so on.Then,our study systematically organizes the clinical application roles of Zhongjing diagnosing and treating diarrhea,which mastering his method helps guide the clinical application.Methods:The methods of document arrangement were used in this study,which combed the lines referring to diarrhea in "Treatise on Febrile Diseases" according to the six meridian syndrome differentiation in order.Through the induction and arrangement of the lines involving diarrhea,the inheritance and development of the rules of Zhongjing diagnosing and treating diarrhea of medical practitioners in later generations,and combining with proved cases by using classical prescription to treat diarrhea in modern,we dig out the clinical application rules of Zhongjing diagnosing and treating diarrhea,which reflects the characteristics of Zhongjing treating diarrhea by six meridian syndrome differentiation.Results:The diarrhea in "Treatise on Febrile Diseases" includes diarrhea, dysentery,and cholera.According to six meridian syndrome differentiation,diarrhea in Sanyang is mostly excess-heat syndrome, which its symptoms are hot smelly stool,anal with burning heat sensation,fever,thirsty,dark yellow urine,yellow tongue fur and rapid pulse and so on,while diarrhea in Sanyin is mostly deficiency-cold syndrome,which its symptoms are cold loose stool,stool with indigested food,feeling cold without fever,not thirsty,clear and copious urine,cold limbs,white tongue fur,deep and slow pulse and so on.The occurrence of diarrhea is related to physical factors,affections by six exogenous pathogens,internal injury by seven emotions (stagnation of liver-qi),irregular intake of food or intake of unclean food, delayed treatment or mistreatment and so on.The character of diarrhea belongs to cold,heat,or mixed cold and heat.It belongs to deficiency, excess,or coexistence of deficiency and excess.The location lies in intestine,but it has relationship with spleen,stomach,kidney,lung,liver, gallbladder,sanjiao,heart and so on,which dysfunction of these Zangfu organs can cause diarrhea.Sanyang diarrhea belongs to excess,heat,which is caused by exogenous pathogen going through skin to Zangfu organs,dysfunction of large intestine conveying and transforming the waste products,unable of discrimination of nutrient and waste products.The characteristic of it is abdominal pain,diarrhea,thirsty,dark scanty urine,white thin fur or yellow fur on tongue,floating and rapid pulse or slippery and forceful pulse,which lies in Fu organ.Sanyin diarrhea belongs to deficiency,cold, (except for diarrhea in Shaoyin disease with heat transformation and diarrhea in Jueyin disease) which is mostly caused by exogenous pathogen going through Zangfu organs or exogenous pathogen causing diarrhea that leads to insufficiency of both the Spleen-Yang and Kidney-Yang.The characteristic of it is loose stool,stool with indigested food,pale tongue with white fur,cold limbs,deep,thready and weak pulse or faint pulse,which lies in Zang organ.Sanyang diarrhea is mostly the excess of pathogens while the vital Qi is not insufficient relatively, which the trend of disease is slight,and the course of disease is short.If the disease can be treated rapidly and correctly,it can be cured soon. Sanyin diarrhea is mostly insufficiency of the vital Qi,while the excess of pathogens is still,which the trend of disease is serious,and the course of disease is long,especially Shaoyin diarrhea and the Jueyin diarrhea are the most urgent.If the disease can be treated rapidly and correctly,it can be cured hopefully.Otherwise,If the disease is delayed or mistreat,the prognosis is bad.Aiming at different pathogenesis,therapeutic methods were summarized such as expelling exogenous pathogen and ascending qi(Ge Gen Tang),cleating heat and expelling exogenous pathogen(Ge gen Qin Lian Tang),cleating heat(Huang Qin Tang,Bai Tou Weng Tang), purgation(Shi Zao Tang,Xiao Cheng Qi Tang,Da Cheng Qi Tang,Da Chai Hu Tang,Chai Hu adding Mang Xiao Tang),inducing urination(Zhen Wu Tang,Wu Ling San,Zhu ling Tang,Fu Ling Gan Cao Tang).warming Zhongjiao and eliminating dampness(Li Zhong Tang, Wu Zhu Yu Tang),warming yang and nourishing kidney(Si Ni tang and so on),nourishing Yin and clearing heat(Zhu Fu Tang),clearing heat of Shangjiao and dispelling cold of Xiajiao(Gan Jiang Huang Qin Huang Lian Ren Shen Tang,Ma Huang Sheng Ma Tang),acrid to diffuse and bitter to descend(Xie Xin Tang and so on),soothing liver and relieving depression(Si Ni San),acid to converge(Wu Mei Wan),inducing astringency and checking diarrhea(Chi Shi Zhi Yu Yu Liang Tang,Tao Hua Tang),and acupuncture and moxibustion(yang syndrome suit for acupuncture and yin syndrome suit for moxibustion).The Chinese herbs such as clearing heat:Huang Lian,Huang Qin,Bai Tou Weng,Qin Pi; warming yang:Fu Zhi,Gan Jiang,Xi Xin;inducing diuresis:Fu Ling, Bai Zhu;ascending qi:Ge Gen,Sheng Ma;invigorating qi:Bai Zhu,Ren Shen;inducing astringency:Chi Shi Zhi,Yu Yu Liang;purgation:Mang Xiao,Da Huang;regulating qi:Chai Hu,Zhi Shi.Conclusion:Our study concluded that:Zhongjing regarded yin,yang,biao,li,cold, heat,deficiency,and excess as program to treat diarrhea based on six meridian syndrome differentiation,which had a more comprehensive understanding of diarrhea.That laid a good foundation for later generations to diagnose and treat diarrhea.
