| Objective :This study was to relationship between the dissected lymph node number and the prognosis in D2 Lymphadenectomy for advanced gastric cancer.Methods: From January 1999 to May 2003, 361 patients with advanced gastric cancer undergone gastrectomy with D2 lymphadenectomy in our hospitl were followed—up in terra of prognostic benefit.Results :In the whole group lymph—node metastasis rates was 82.8%,the lymph—node metastasis rates of the U,M and L were 85.5%,83.1%,80.1% in the different sites and the T2,T3,T4 were 53.85%,77.22%,94.19%in the different depth of invasion of gastric cancer and the stage II,III,IV were 24.0%,88.7%,100.0%. The 5-year survival rates was 34.1% in the whole group,the 5-year survival rates of the U,M,L were 30.4%,33.9%,37.6% in the different sites and the T2,T3,T4 were 69.2%,41.1%,20% in the different depth of invasion of gastric cancer and the stage II,III,IV were 62.0%,32.1%,23.2%. Among those patiens with the same site,depth of invasion and stage, the more lymph nodes resected , the better survival outcomes achieved, with greatest comparative discrepancies.(Log-rank trend test P<0.05).Conclusions: Dissection of suffcient lymph nodes in the procedure of D2 lymphadenectomy for advanced gastric cancer should be recommended so as to improve the long-term therapeutic effects, however the site,depth of invasion and stage are. |
