| Objective1. Understand the situation that growth hormone receptor (GHR) and insulin-like growth factor-1 (IGF-1) expression in colorectal tissues of patients with colorectal cancer.2. Explore the relationship between GHR and IGF-1 expression in colorectal cancer and clinicopathological factors.3. Clarify correlation of GHR and IGF-1 expression in colorectal cancer.4. Explore the independent influencing factor of impact GHR and IGF-1 positive expression rate in colorectal cancer.5. Explore the interaction between smoking, drinking and GHR, IGF-1 expression in colorectal cancer.6. Analysis and evaluate the role of GH - IGF-1 axis in colorectal cancer progress.Methods1. This study included 154 protopathic patients with colorectal cancer, who hospital at No.8 hospital in Wuhan from November 2004 to May 2007. Immunohistochemistry technique was used to detect GHR and IGF-1 expression.2. Through access to the medical record and return record collect the relevant information, this information includes the contents concluded demographic feature, clinical feature, pathological feature, clinical laboratory information and environmental exposure data.3. The data was analyzed with SPSS 12.0 software: chi square test was used to compare the positive rate of GHR and IGF-1 expression; Spearman rank correlation was used to analysis the correlation of GHR and IGF-1 expression in colorectal cancer; Through non-condition Logistic regression model screen the independent factors which influence the possibility of GHR and IGF-1 expression in colorectal cancer; according the principle of case-series study evaluate the interaction of smoking, drinking and GHR,IGF-1 expression in colorectal cancer.Results1. Positive rates of GHR in colorectal cancer tissues, adjacent tissues, and normal tissue were: 69.3%, 54.8%, 45.9%, positive rate of GHR express in three kinds of the tissues were different. The Positive rates of IGF-1 in colorectal cancer tissues, adjacent tissues, and normal tissue were: 76.8%,64.5%,64.9%, The positive rate of IGF-1 express in colorectal cancer tissues is different from the positive rate express in adjacent tissues, and normal tissue, but IGF-1 expression in normal tissue and adjacent tissues was no difference.2. The clinical pathology factors associated with GHR expression were Tumor location, tumor diameter, the Ducks classification, lymph node metastasis, three tumor marker Combined test results, smoking history, whether suffering from colorectal polyps.3. The clinical pathology factors associated with IGF-1 expression were whether suffering from hypertension, the Ducks classification, lymph node metastasis.4. GHR, IGF-1 expression of different tissues in the same patient is a positive correlation. There is a certain degree correlation between GHR and IGF-1 expression of the same patient.5. In logistic regression model, Tumor location is the only influence factor of the GHR expression probability in the normal colorectal tissues. Positive expression of GHR in normal colorectal, Lymph node metastasis and whether suffer from polyps promote GHR expression in adjacent tissue. Influence factor of GHR express in cancer tissues are age and whether the tumor greater than 3 cm in diameter.6. The positive expression of GHR in normal colorectal tissue and age are independent influence factors of IGF-1 in normal colorectal tissu. The influence factors of IGF-1 expressiom in adjacent tissues are IGF-1 expression in normal tissue, tumor location, as well as lymph node metastasis. Drinking and IGF-1 and GHR expression in normal tissue are influence factors of IGF-1 expression in colorectal cancer.7. Only in the adjacent tissues GHR expression exist interaction with smoking status.Conclusion1. GH, IGF-1 expression is an important event to promote tumor growth in the process.2. Lymph node metastasis in patients with colorectal cancer related with GHR and IGF-1 expression in adjacent tissue, GH, IGF-1 can promote tumor metastasis.3. Smoking can contribute to adjacent tissue GHR expression in patients with colorectal cancer, and become risk factor of tumor lymph node metastasis in colorectal cancer.4. The probability of IGF-1, GHR expression in patients with colorectal cancer, in addition to influenced by the Individual difference of IGF-1, GHR expression, but also by the patient's age, tumor location, size of tumor, lymph node metastasis, drinking, whether suffering from Colorectal polyps .5. GH-IGF-1 axis plays an important role in colorectal cancer occur and process. |
