Objective: Nasalpolyp (NP) and Nasalpolyosis are common and frequently occurring diseasies in otorhinolaryngology head and neck surgery, the pathogenesis of which remains unclear. Several hypotheses have been put forward regarding the underlying mechanisms including chronic infection, aspirin intolerance, alteration in aerodynamics with trapping of pollutants, epithelial disruptions, epithelial cell defects/gene deletions (CFTR gene), inhalant or nasal allergy . It is important to remember that there is no single etiological factor that is responsible for the development of nasal polyposis, but that inflammation still remains to be the central major factor in all nasalpolyps.Nasalpolyposis and NP are chronic inflammatory disease of the upper airway characterized histologically by the infiltration of inflammatory like eosinophils or neutrophils. In majority of nasal polyps, eosinophils comprise more than 60% of the cell population, except in cystic fibrosis. Cytkine -chemokine cascade results in increased inflammatory cell infiltration(predomin antly eosinoiphils).The migrated eosinop hils can in turn further enhance the inflammatory process by contributing to stromal fibrosis, epithelial damage, increased interstitial edema,and increased extracellular matrix production. To study the expression of rantes and Eotaxin in nasal polyp osis and nasal polyp and explore the different mechanism bet ween polyposis and polyp.Statistics management Kruskal- Wallis test and Nemenyi test were used to analyze the data with the significant difference ofα=0.05.Method:Choosing cases,grouping and obtaining specim ens Dividing to three groups: nasal polyposis , nasal polyp and septal deviation . 17 Polyp tissues of nasal polyposis were obtained from the patients (males 11, females 6: age range 38一72 years,average age 52.5 years) who were subjected to nasal Polypeetomy in our department . 13 Polyptissues of nasal polyp were obtained from the Patients (males 8,females 5: age range 13一58years,average age 38.5 years) who were subjected to nasal Polypeetomy in our department too . Inferior turbinate mueosa as control group were from 9 Patienis (all male,age range 22一52 years,average age 32.5 years) with rhino一genesis snoring. All cases have not the history of asthlna,allergy,aspirin intolerance and not receiving steroids before two weeks. All specimens were fixed with 4% Paraformaldehyde , Paraffin一embeded and make sections at 5 um thick. The slides were detected with HE staining for routine histopathologic examination , immuno- histoehemieal staining was performed for observing the expression of rantes and eotaxin. Result :1 Nasal Polyps tissue is morphologieally charaeter ized by oedema,goblet cell hyperplasia of the epithelium,is extreme edema and gland proliferation.There are a lot of inflammatory cells in nasalpolyp tissues by HE. Eosinophils in Nasal Polyps tissue were higher than in controls.2 In nasal polyps group, rantes expressed,Widespread in the mucosa epithelial, the extracellular matrix with infiltrated inflammation cells and the vascular endothelial.①The expression of rantes in nasal polyosis and polyp were dramatically higher than in controls (P< 0 . 05) , the expression of rantes between nasal polyosis and polyp was no remarkable difference (P> 0 . 05)3 Eotaxin expressed,Widespread in the mucosa epithelial, the extracellular matrix with infiltrated inflammation cells and the vascular endothelial too.The expression of Eotaxin in nasal polyosis and polyp were dramatically higher than in controls (P < 0 . 05),the expression of Eotaxin between nasal polyosis and polyp was no remarkable difference (P> 0 . 05) .Conclusion :It is a important pathological characteristic of nasal polyposis and nasal polyp a great many eosinophils recurit into the nasal mucosa and it was considered as a critieal joint resuting from the diseases . It do good to the contorl of nasal polyposis and nasal polyp to inhibit the recruitment and activation of eosinophils. It is a complicated biological proccess that eosinophiles is recurited into the local tissues,which involve many cytokine,adhesion molecule,chemokine and inflammation mediator.Eotaxin and rantes are two factors which selectively attract and activate eosinophils were considered as a non replaceable factor in this part because of their hightest selectivity and effieieney. Eotaxin and rantes bind with the specifical receptor CCR,consequently play biological role.Eosinophils recruitment in the tissue is orehestrated by chemokine especially eotaxin and rantes to a great extent.In a word , Eosinophils infiltration may play a prominent role in the development and recurrence of nasalpolyps and polyp. Rantes and eotaxin actively participate in the process of eosinophilic inflamat ion .Both of rantes and Eotaxin likely play key roles in the inflammatory reaction process of nasal polyosis and polyp . Thereby, it can provide theory roof for curing and preventing nasalpolyps and polyp in the future. |
