The Difference Of The Family Gout And Sporadic Gout

Purpose:By studying the difference of the clinical features between family gout and sporadic gout,to provide the clinical signs for guiding the individual therapy.Method:gather 73 cases of family gout patients and 243 cases of sporadic gout patients,make a contrast between the two on sex ratio,age of onset,metablic syndrome ratio,the precipitating factor,position,duration and emission way of paroxysm,and prognosis.Result:among the 73 cases of family gout,93.25%of them are male.Among the 243 cases of sporadic gout patients,91.15%of them are male.Comparing with these two, P=0.5094,there are no statistical significance.The age of onset of family gout patients is 44.36±14.55,while that of sporadic gout is 51.09±13.30,comparing with the two, p=0.0016,there are statistical significance.Among the family gout patients,82.07%of them are metabolic syndrome patients.Among the sporadic gout patients,(80.53%) of them are metabolic syndrome patients,comparing with the two,p=0.8114,there are no statistical significance.Among the 73 cases of family gout patients,63 of them are caused by drinking and(or) seafood.Among the 243 cases of sporadic gout patients,170 of them are caused by drinking and(or) seafood,comparing with the two,P=0.0253,there are statistical significance.48 cases of the family gout patients have a night attack,while for the sporadic gout patients,168 of them have a night attack,and there is no statistical significance.44 cases of the family gout patients are in first metatarsophalangeal joint, while for the sporadic gout patients,170 of them are in first metatarsophalangeal joint, P=0.7344,there are no statistical significance.The duration of pain for family gout patients are 12.15±24.83 days,while that of sporadic gout patients are 9.28±17.80 days, P=0.3409,there are no statistical significance.40.80%of the family gout patients are in acute exacerbation,it could be eased by colchicines,while for the sporadic gout patients, 37.61%of them could be eased by colchicines,P=0.7285,there are no statistical significance.44.70%of the sporadic gout patients could be eased after taken non-steroid anti inflammatory drug,while 39.38%of the family gout patients could be eased after taken non-steroid anti inflammatory drug,P=0.5466,there are no statistical significance.50.07%of the family gout patients have the deeper trend,42.04%of the sporadic gout patients have the deeper trend,p=0.031,there are no statistical significance. The systolic pressure of the family gout patients are 136.7±22.53 mmHg,while that of the sporadic gout patients are 136.3±17.90mmHg,p=0.8919,there are no statistical significance.The self monitoring blood glucose of the family gout patients are 5.66±1.18mmol/L,while that of the sporadic gout patients are 5.86±1.60mmol/L, p=0.4047,there are no statistical significance.The Total cholesterol of the family gout patients are 4.29±2.30mmol/L,while that of the sporadic gout patients are 4.91±1.25mmol/L,p=0.0114,there are statistical significance.The triglyceride levels of the family gout patients are 3.07±1.60mmol/L,while that of the sporadic gout patients are 2.02±1.36mmol/L,p=0.062,there are no statistical significance.The Urine pH of the family gout patients are 551.3±123.5mmol/L,while that of the sporadic gout patients are 524.9±133.8mmol/L,p=0.0243,there are no statistical significance.The URIC uAER of the family gout patients are 4.12±1.19%,while that of the sporadic gout patients are 3.94±2.06%,p=0.9288,there are no statistical significance.14.67%of the family gout patients have kidney stones or Cyst of Kinney,while 9.3%of the sporadic gout patients have kidney stones or Cyst of Kinney,P=0.3184,there are no statistical significance.Conclusion:after the contrast between family gout patients and sporadic gout patients:The male accounted for a vast scale in both,and there is no statistical significance.The average age of onset of family gout patients is obviously earlier than that of sporadic gout patients.Both family gout patients and sporadic gout patients are easily having metabolic syndrome,but family gout patients are more easily get disturbance of lipid metabolism.For the ratio of causing by drinking and PURINE diet,family gout patients is obviously higher than the ratio of sporadic gout patients.Both family gout patients and sporadic gout patients are easily get a night attack,and both attack firstly in first metatarsophalangeal joint.The Urine pH of the family gout patients is obviously higher than that of sporadic gout patients,but there is no obvious difference on the URIC uAER between the two.Both family gout patients and sporadic gout patients can be eased by using colchicines and non-steroid anti inflammatory drug when attacking.
