Value Of Pathologically On Defining The Gross Tumor Volume Of Speculation In Small Peripheral Lung Carcinoma

Objective:To study benign and malignant characteristics of small peripheral lung cancer and the value of outlining three-dimensional conformal radiotherapy target through pathological big section.Methods:we choose 7 Small Peripheral Lung Carcinoma specimen(four adenocarcinoma and three squamous-celled carcinoma specimen) after operation which have rich speculation confirmed by spiral computed tomography,and then mended these specimens fairly.We poured stationary liquid into these specimens,proceeded dehydraction and waxdip in full centrol of access dewaterer,lastly,we chiped slice and dewration.we observed the pathological big section in low and high power microscope.To count benign and malignant of speculation according to its morphological characteristic by short or long and thick or thin.Spiculation is divided into long and short by 5mm,thick and thin by 2mm of the basis pontis width.Rusults:The number of speculation is 127,malignant spiculation is 92 and benign speculation is 35 of the total.1.The number of short speculation is 96 and malignant spiculation is 83 in them.The number of long speculation is 31 and malignant spiculation is 9 in them.The length of spiculation with benign and malignant have statistical significance(χ~2=38.70,P＜0.01).2.The number of thick speculation is 33 and malignant spiculation is 14 in them.The number of thin speculation is 94 and malignant spiculation is 78 in them.The basis pontis width of spiculation with benign and malignant have statistical significance(χ~2 = 20.13,P＜0.01).3.The number of adenocarcinoma speculation is 85 and malignant spiculation is 64 in them.The number of squamous-celled carcinoma speculation is 42 and malignant spiculation is 28 in them.The adenocarcinoma and squamous-celled carcinoma of spiculation with benign and malignant havn't statistical significance(χ~2=1.048, P=0.306＞0.05).4.The number of short and thin speculation is 82 and malignant spiculation is79in them.The number of thick and long speculation is 19 and malignant spiculation is 4 in them.The thin-shortly and thick-longly of spiculation with benign and malignant have statistical significance(χ~2=54.13,P＜0.01). Conclusions:The majority of thin-shortly spiculation were malignantly,When we outline the real target volume,the spiculation which length shorter than 5mm and width less than 5mm should be included in the gross tumor volume.
