Objective To discuss the possibility of the application etomidate and remifentanil in total intravenous anesthesia,the comparison of effects in hemodynamics between the use of etomidate and propfol.Furthermore,the optimal usage of etomidate will also be discussed in this essay.Methods:Forty patients scheduled for LC operation for gallbladder were divided into groups of 20 each:etomidate group(groupE) and propofol group(groupP).The two groups have the anesesia induction as:first,injected scopolamine 0.3mg and fentanil 3μg/kg intravenously 5 minutes before induction;second,injected vecuronim 0.15mg/kg,etomidate 0.3mg/kg for group E and propfol 2mg/kg for group P.The patients in groupE was maintained with 0.6~1.2mg/kg·h-1 etomidate,and the patients in goupP was maintained with 6~10mg/kg·h-1 propofol.The both groups were combined with remifentanil 5~15μg/kg·h.Observe ardiovascular effects before anesthesia,1min before trachea intubation,1min after trachea intubation,3min after cutting skin,3min aider exploration,3min after cutting ill focus,3min after tracheal extubation,30min after operation.At the same time,observed analepsia condition.Results:The group E has a better hemodynamics before intubation than group P.However, 10min after cutting ill focus blood pressure is not satisfited in group E.Both group E and group P can reach requirement of TIVA in LC operation.The time of analepsia in group E is longer than that in group P.Conclusions:The use of etomidate in TIVA is proved that it could maintain the blood pressure and the heart rate at a stable level.There is no significant difference in the result between etomidate and propfol.The satisfactions of patients are reasonably high.However,the regaining consciousness time of etomidate group is longer than propfol group. |
