Comparing Systematic Ultrasound Guided Versus Contrast-enhanced Targeted Biopsies: Impact On Prostate Cancer Detection

Objective:We performed a prospective randomized trial comparing ii transrectal ultrasound(TRUS) guided systematic biopsy(SB) cores to contrast-enhanced ultrasoud(CEUS) targeted biopsy cores to determine the impact on the cancer detection rate.Search for the better biopsy method to increase positive rate.Method:We prospectively randomized 61 screening volunteers with an elevated PSA,suspected prostate cancer by digital rectal examination, serum prostate-specific antigen(PSA) and transrectal ultrasonography to undergo contrast-enhanced targeted or SB,including patients of other imaging examinations suspected PCA,or were found metastatic tumors which doubt originating from prostate cancer.However,except for patients of contraindication in biopsy and ultrasound contrast agent.Then, histopathological examination preforms After ultrasound -guided biopsy.Results:1,All 61 the histologie study proved 19 prostate cancer,of which detection rate is 31.15%,3 atypical hyperplasia,4.92%,36 benign prostatic hyperplasia,59.02%except for 3prostatitis,4.92%.2.Cancer is detected in 8/30 patients(26.67%) with SB,and in 11/31 subjects (35.48%) by targeted biopsy.The cancer detection rate is not significantly better for the targeted approach(P＞0.05,McNemar).All 31 patients in targeted biopsy,there are 257 organizations including 29 caner organizations.All 30 patients in SB biopsy,there are 330 organizations including 21 caner organizations.The detection rate for targeted biopsy cores(11.28%,29/257) is better than for SB cores(6.36%, 21/330),P＜0.05,McNemar.Conclusion:1,The cancer detection rate is not significantly better for the targeted approach.2,The detection rate for targeted biopsy cores using much less cores is better than for SB cores.
