Study On The Present State And The Influencing Factors Of The Cesarean Section In The Three Gorges Region Of Chongqing

Cesarean section is one of the important ways to treat dystocia. In recent years, the cesarean section rate had been raised, with the technology progress of cesarean section, the advancement of anesthetic and antibiotic application, and many social factors, which had bring problems being worth of thinking to the maternal and children's health and the social economy. The increasing cesarean section rate has some positive actions to solve pregnancy complications,decrease the mother and fetal morbidity and mortality rate in the perinatal stage, however the cost of hospitalization and the operative complications increase corresponsively. If we continue to raise the rate blindly to some extent, anoxia rate of newborn is not reduced, and there is a increasing relative risk of the maternal mortality also. The main objectives of our research are: To analyze the present state and the influencing factors of the rising cesarean section rate, aimming directly at it,we'll produce the measures to control and reduce the rate of cesarean section.The object of this study is the population of the Three Gorges Region of Chongqing. It had retrospectively analyzed datas based on partial patients delivered in hospital(3278) in 2000 and 2006 of Fengjie county,Zhongxian county and Pengshui county. With questionnaire pattern, we investigated the delivery style and the reasones of choosed it, the knowledges and attitude to the cesarean section of the lying-in women and their families in 12 midwifery hospitals of these three counties. In addition, we interviewed the regulators and the obstetrics staff in midwifery hospitals, the parturients and her family members concerning the cesarean section rate. It also analyzed initially the current situation of cesarean section and its influencing factors in Chongqing.These results were made clear:1. There was a high and increasing cesarean section rate in the Three Gorges region recently. It had increased from 44.19 % in 2000 to 59.43 % in 2006. 2. Comparing with vaginal delivery, cesarean section has some defects below: the volume of postpartum hemorrhage in 2 hours had been increased(48.9ml), average stay's elongation(1.1d), aggravated economic burden, and the beginning of breast feeding had been delayed(4.1h).3. The influential factors involve two parts: medical factors and social factors. The medical factors include fetal distress, cephalopelvic disproportion, complication in pregnancy and gestational weeks. The social factors include age, occupation, way of pay the cost of hospitalization, knowledge of cesarean section and the medical workers' hold tolerational attitude to it , et al. The society factor become the principle reason of cesarean section in recent years.4. According to the progress of correlated study and current situation of this research, the researchers provided some strategies and suggestions to control and reduce the cesarean section rate.Government should pay close attention to the rising cesarean section rate and think highly of it. We should reinforce the training of the staff of obstetrics, increase the service quality of perinatal care in countryside, change the service mode of obstetrics and spread the suitable techniques. Meanwhile, we should to develop the health education and provide psychological support to the pregnant women. It can increase the awareness of health to the pregnant woman and her family members and make them to choose the delivery style scientific and reasonable.
