Transmission of HIV through blood donation and transfusion is one of the major routes for the AIDS epidemic in China. In the mid to late 1990s, a severe HIV/AIDS outbreak was identified among plasma/blood donors in Henan and neighboring provinces in central China, which has caused serious and profound social problems. The Chinese government has taken a confluence of anti-HIV prevention and intervention measures since 1998, and as a result, has successful controlled the epidemic in this population. Although health authorities and institutions have carried out a number of epidemiological studies about the spread of the AIDS epidemic through commercial blood donations and transfusion, disputes remain in the real causes of blood donation and the effects of social and historical environment on its occurrence and development, which deserve further studies.Through literature review, this paper depictsa real picture of the AIDS epidemic in Henan province by reviewing relevant national and local official health records on commercial blood collection and transfusion in the late 1980s and 1990s. From a historical perspective, the paper shows the real development map of the AIDS epidemic in Henan province by analyzing its social, economic, cultural environment, and the formulation and implementation of related health policies and measures.This study indicates that the AIDS epidemic in Henan province occurred in a particular social and historical circumstance. It represents a mass of emerging problems in various aspects of an area like Henan or a country like China during a transitional status. The HIV/AIDS epidemic was just one of many public health problems and challenges for China at that time. The world is still lacking appropriate knowledge and technology for preventing and control the AIDS epidemic. The destruction of public health infrastructure and shortage of effective health administration have brought a great deal of public health problems. It is only through the government's continuous efforts on maintaining economic stability and development, improving people's living standard, taking effective preventive measures that population health can be improved.
