| Objective:To evaluate the risk factors affecting the survival rate and survival time for hepatocellular carcinoma(HCC) and to compare 5 available prognostic staging systems,including AJCC/UICC staging system,Japan Integrated Scoring(JIS) system, Cancer of the Liver Italian Program(CLIP) scoring system,Barcelona Clinic Liver Cancer(BCLC) staging classification and China staging(CS) system in patients with HCC.Methods:The clinical data of 51 cases of patients who had been diagnosised as hepatocellular carcinoma in our department from April 2004 to April 2007 were retrospectively analyzed.Every potential prognosis factors were collected and analysed by chi-square test and COX regression method.The survival of those patients with respective to different stages were analyzed and compared using Kaplan-Meier curves and Log rank method.Results:Every patients have been performed certain surgical treatment.By the single factor chi-square test,5 factors such as the tumor size and number,the tumor thrombosis,metastasis and the serum albumin show an important significance on the prognosis of HCC(P<0.05).And by the multiple factor regression method,the tumor number,tumor thrombosis and the serum albumin also show a stronger significance on the prognosis of HCC(P<0.05).The survival curves and test shows the JIS system and the CS staging system are comparably more efficient in predicting the prognosis of HCC patients.TMN staging system and CLIP system also have their value in predicting the prognosis and the BCLC classification show a poor ability of predicting.Conclusions:The tumor's number,tumor thrombosis are the most important prognosis factors of HCC.The serum albumin,AFP also have a relationship with the patient's survival rate and time.JIS system and CS system both contain the two important factor of HCC which is the tumor factor and the liver function.For this reason they both have great value in predicting the prognosis of HCC patients and can be useful in clinical treatment of HCC patients. |
