Background:Modern medicine has focus on chronic disease.Asthma is a most common chronic illness in childhood.The increasing prevalence of asthma in children has become a major worldwide public health problem in recent decades.Of chronic diseases,asthma remains an important cause of absence from school for children and absence from work for their parents.It also influences mental growth for children,and the influences will be lasted when they grow up.Asthma produces a substantial economic burden,including both direct and indirect costs.Despite a considerable literature on risk factors for asthma onset and severity in children,very little is known about possible influences,particularly how these factors interact with asthma.The recent studies have identified multiple riskfactors for the development and progression of the asthma.As a result,much research is focused on identifying univariate that can be initiated at asthma and prevent disease progression.Asthma is a complex disease with a strong heritable component and multiple environmental influences and immunological influences and psychological influences and the other influences.However,seldom studies have done in asthma for multivariable risk factors.The objective of this study was to determine the relationship with multivariable risk factors and asthma.Objective: The aim of this study was to find out the important risk factors of children asthma in the city zone of QingDao, and to provide scientific basis of prevention and control to the doctor and the health administration. Thus to decrease the incidence of children asthma and improve the children's survival quality.Methods:A questionnaire survey was conducted on the children who enter in the Affiliated Hospital of The Medical College of Qingdao University during May 2005 to April 2006 ,The matched case-control study was performed in 300 children with asthma and 300 cases of the matched controls,0 to 14 years of age.The diagnostic of asthma was made through Chinese Journal Of Pediatrics(2004),Asthma was designed based upon case history,family history,atopy,the factors of parents,environmental factor,psychological factor,et al.To access the relationship of factors to asthma,we used univariate and multivariate logistic regression model which was employed to evaluate the risk factors for asthma.The dependent variable was presence and absence of asthma.Statistical testing was done at the two-tailed alpha level of 0.05,odd ratios(OR)and 95%CI was calculated.Data were analyzed using the SPSS 11.5 software. Results:Three hundered mathed pairs were investgated, the ratio of males to females was 1.56:1 .The result of univariate analyses indicate the statistically difference in asthma that: parents'culture, maternal passive smoking during pregnancy,children's passive smoking, breast milk,disease of parents in respiratory system, income of family, atopy (inclding rhinitis or/and eczema or/and urticaria) ,acute respiratory infections, heavy exercise , eating habit , cuckoo products, family fitment , bask in the winter, exhaust fume in kitchen, effectiveness of exhaust fume, breeding house pet , rearing flower,family history of asthma,family history of atopy, family history of food allergy, dust allergy,systemic therapy after the first attack ,often depressed,bellowing,uncomfortable with no reason, positive result of SPT had statistic influence on childhood asthma..The tendency still existed after excluding all the possible confounding factors in the multivariate logistic regression model,in addition, parents'culture, maternal passive smoking during pregnancy,children's passive smoking, breast milk, acute respiratory infections, heavy exercise, disease of parents in respiratory system, income of family, atopy (inclding rhinitis or/and eczema or/and urticaria) , eating habit , family history of asthma,family history of atopy, breast milk, family fitment, breeding house pet, often depressed,bellowing,uncomfortable with no reason, systemic therapy after the first attack, positive result of SPT.Conclusion: parents'culture, maternal passive smoking during pregnancy ,children's passive smoking, acute respiratory infections, heavy exercise, family fitment, disease of parents in respiratory system, income of family, atopy (inclding rhinitis or/and eczema or/and urticaria), eating habit, family history of asthma,family history of atopy, often depressed,bellowing,uncomfortable with no reason, positive result of SPT were the important risk factors of children asthma. Among these risk factors,recurrent respiratory tract infection became the chief risk factor.The breast milk, breeding house pet and systemic therapy after the first attack were the protective factors of children asthma. Improvement and avoidance these risk factors,control these risk factors is very important to the prevention of occurrence and severe degree of asthma in children. |
