Objective:To investigate the therapeutic effects of pin hole multi-directional intracranial hematoma aspiration in severe intracerebral hemorrhage and discuss its operation methods.Methods:Objects of study are patients with severe intracerebral hemorrhage.,They are treated with head CT inspection in the hospital,。we can work out the volume of hemorrhage and the central line structure shifting degree according to the CT., Immediately operate with those who cohere with operation standard with pin hole intracranial hematoma aspiration。and sets at the tube drainage,Treat with CT again after 24 hours,and the aspiration from top and bottom,front and back according to the residual volume of hematoma,the place of tube and horizontal bedding of CT,treat with CT again after 24 hours,and according to the residual volume of hematoma to decide whether treat with aspiration or not,general patients can pull out the tube after 2~3 times.And it can stop if the residual volume of hematoma is smaller than 15ml. The comparison group patients are stochastically treated with conservative medical treatments,assign mark on the two groups about the dead rate,GCS,nerve function damage,compare with the residual volume of hematoma and the central line structure shifting conditions.Results:1.To assign mark on the two groups about GCS and nerve function damage。20 patients are alive and 10 are dead on the 10th day in the treatment group.The GCS are 6.13±0.81 on the 1st day,9.33±1.47 on the 3rd day,and 10.00±1.46 on the 15th day.The marks on nerve function damage are 37.83±1.31 on the 1st day,25.65±1.84 on the 15th day,±and 12.65±1.40 in the 3rd month.The comparison group:8 are alive and 20 are dead on the 10th day.The GCS are 5.18±0.40 on the 1st day,6.31± 1.13 on the 3rd day,and 10.44±1.93 on the 15th day.The marks on nerve function damage are 40.93±6.0 on the 1st day,22.63±0.84 on the 15th day,and 12.50±0.52 in the 3rd month.There is not a statistics meaning in the comparison.(P>0.05)2.The effects of hemorrhage and central line structure shift to the cured patients.The treatment group are following as(ml),(cm),64.14+6.37 and 0.98+0.14on the 1st day, 11.00+0.73 and 0.38+0.10 on the 10th day.The comparison group are 60.27+6.95 and0.84+0.14;48.75+3.59 and 0.63+0.16.Before and after the operation,there are statistics meaning in hemorrhage and central line structure shift and the comparison to the comparison group.3.Treatment effective:Effectiveness of the treatment group is 66.67%,the dead rate is 33.33%.Effectiveness of the comparison group is 28.51%,the dead rate is 71.43.%Conclusion:The pin hole multi-directional intracranial hematoma aspiration can effectively eliminate the hematoma and recover the central line structure early,and reduce the dead rate,and there are not remarkable effects to the GCS and nerve function damage of the alive patients.
