To Study The Incidence Of Bacterial Vaginosis In Six Units Of Work In Changchun City

Objective:The main factor of this text is the incidence of BV in the 6 units of work.To study and discuss the incidence fo BV of common female in Changchun City as well as the correlated reasons and characteristics of BV.Stuff and methods:Select 2718 cases of women aged from 20 to 60-year-old,who came to our hospital for health examination from August 2008 to January 2009.Ask and record personal information carefully,including career,menstrual history,method of contraception,habit of vaginal washing,taking mediciens or not,having any clinical symptom or not(pruritus vulvae,increasing discharge,smelly) and so on.The subject arrange lithotomy position,and doctor open her vagina using a vaginal speculum,check the colour , character and quantum of vaginal discharge.Collect discharge from the up 1/3 of vagina using a aseptic cotton swab.1.check the pH of vagina:Directly contact a piece of pH test paper with the discharge on the cotton swab for harf a secone,compare it with the standard shade selection card after 30 second,read and record the result.pH>4.5 is helpful for diagnosis of BV.2.Smear the discharge on a glass slide,add saline 1-2 drops,put a cover glass on it,then look for clue cells under the 400times microscope directly.Clue cells are epithelial cells with irregular and fuzzy borderline.The proportion of clue cells in epithelial cells≥20% is clue cell positive.3.whiff test:Smear discharge on another glass slide,add 1-2 drop KOH solution on it,if there is a pungent smell of ammonia,it means whiff test positive,or else whiff test negative.according to Amsel method,if three of the follwoing four are positive,you can diagnose BV:①Vaginal secretion is uniformity,tenuity and easy to clean.②The pH of vaginal>4.5.③Whiff test positive.④clue cell positive.Choose BV patients,calculate the incidence,and compare their age,career,living habit and contraception method,analyze impact of these factors on BV.results and discuss: Bacterial vaginosis(BV) is a common and complicated gynecological syndrome, charact- erized by the changing of the normal vaginal flora proportion.The pathogenesis of BV is reduction of the number of lactobacilli in vagina caused by any reason,meanwhile,other pathogens,which can produce a lot of amine,proliferate sharply.BV can revoke severe complications of the female reproductive systom,including trachelitis and endometritis,it can also increase the risk of HIV infection and premature delivery.This disease is particularly prevalent in women of child-bearing age,the wast majority of patients aged form 15 to 44.It account for 40%～50% of the vulva and vaginal infections,so it has a serious impact on women's reproductive health.The clinical manifestation of BV is a increase of tenuity and smelly vaginal discharge,which can become aggravate after sexual intercourse.Vaginal and cervical examination tells no significant abnormalities.BV bring inconvenience to patients daily life and burden of spirit.What's more,no matter clinical symptoms or not,it could cause many complications and a inceasing risk of HIV infection.There are still about 50% of patients without clinical symptoms,who are easily overlooked.The samples of my investigation is common women who comes for health examination,to study the incidence of BV in common women and correlated reasons of the disase.The study shows that:①There are 159 BV patients among the 2718 samples,it tells the existance of BV patients in the general population.The incidence of the 6 unit of work is 5.85%(159/2718).②The different incidence between different occupations are obviously.The cleaning staff have the highest incidence(9.57%) , and the Power Supply Bureau staff have the lowest one(4.17%),there is statistically significant between them(p <0.01).The reasons may as follows:Power Supply Bureau workers have higher levels of cultural knowledge,they have better work enviroment,less work ,better health habit and take health exmination every year,so incidence of BV is much lower than cleaning staff,whose condition are exactly opposite.The result shows that,career,level of knowledge has obvious impact on the incidence of BV.Women with long work time,long time of sitting position,large work load,lack of education and poor consciousness of personal hygiene have higher incidence of BV,compared with women with short work time,motive job,good education and good personal hygiene.③The survey shows that most BV patients aged from 20 to 30.The incidence of women between 20-30 years old is significantly higher than the incidence of those who are older than 50,there is statistically significant between the two groups(p <0.01).Because 20 to 30-year-old women are sexually active,and BV has elationship with sexual life, they are easier to get BV.The incidences of 30-40-year-old women and 40-50-year-old women are similar,there is no statistically significant between the two groups(p >0.05).④In women who use IUD,the incidence of BV is 9.46%,meanwhile,the incidence of women without IUD is 3.02%,there is statistically significant between the two groups(p <0.01).Many women who use IUD have irregular vaginal bleeding.Long-term vaginal bleeding can change the micro-environment of vagina,heighten the pH of vagina and reduce defense ability.In addition,blood is very good medium for bacterial,it build a suitable condition for dysbacteria,which can lead to BV.⑤Incidence of BV of subjcts who have habit of vaginal washing is11.44%,while the incidence of those who without this habit is 3.78%.There is statistically significant between the two groups(p <0.01).Vaginal washing may kill the lactobacillus in vagina,break down the acidic environment,result in a large number of the proliferation of pathogens,and cause a significant increase of the incidence of BV.Conclusions:①The incidence of BV fo general population in Changchun City is 5.85%.②Age have impact on incidence of BV,women aged from 20 to 30 have the hightest risk of getting BV.③Occupation and level of education have impact on incidence of BV. Women with long work time,long time of sitting position,large work load have higher incidence of BV,compared with women with short work time,motive job.④The incidence of BV of women who use IUD is much higher than incidence of those who don't use IUD.Placing IUD could increase the incidence of BV.⑤The incidence of BV of women who have the habit of vaginal washing is much higher than the incidence of those who don't have the habit. Vaginal washing can raise the incidence of BV obviously.
