The Protective Effects Of Desflurane Pretreatment In Ischemical Reperfusion Injury Of Liver And Its Mechanisms

Objective:We observe the effects of desflurane pretreatment in ischemical reperfusion injury of liver to ascertainment the protective effects of desflurane pretreatment.We will study and research the influence of inflammatory factors , oxygen free radical and inflammatory cells in hepatic tissue with desflurane.We also try to discuss its mechanism.Methods:60 patients scheduled for liver surgery were studied.30 patients received desflurane pretreatment at the beginning for 30 minutes;30 control subjects didn't receive any inhalation anesthetic in whole surgery.ALT,AST,TBIL,CB,albumen,globlin andγ-GT were measured perioperatively.Observation the changes in histomorphology of the liver and activity of ROS.Results: ALT,AST,TB,CB andγ-GT continued to increase in two groups.We found significant differences of ALT contents perioperatively(20min after reperfusion and 1d,3d,5d afteroperation)between two groups.The significant differences of AST only appeared the 5th day afteroperation.There had been significant differences of TB,CB andγ-GT between two groups since induction.No significant differences of albumen and globlin were observed between two groups except the first day afteroperation.The LM observed that there were more inflammatory cells in control group,as cell swelling, vacuolar degeneration, nucelus disappearing and cell necrosis could be seen more often.However,through desflurane preconditioning,the cell structure in this group remains normal.Conclusions: Desflurane preconditioning can lessen the damage of Liver function and the infiltration of inflammatory cells infiltrate,and decrease the production of ROS and its reaction to protect the liver.
