Clinical Application Of Water Flushing Method In Small Incision Cataract Surgery

To evaluate the clinical feasibilities of water flushing method in small incision cataract surgery and to compare with traditional small incision cataract surgery.Old age patients who suffered from cataract surgery in Peng Nian Guang Ming campaign between June and August in 2007were chosen restrospectively. 168 senile cataract sufferers whose hardness of the nucleus core were about III~IV level were chosen. The average age of the sufferers was 69.36±1.42 years old and the average eyesight was 0.1 before the surgery. And the other eye diseases and body factors affecting the operation were eliminated. The 92 sufferers dealing with small incision nonphacoemulsification cataract surgery were set as experiment group (called water flushing group later); the 76 sufferers dealing with traditional small incision nonphacoemulsification surgery were set as comparison group (called routine group later). Both of the two groups shows no significant differences with respect to sex, age, hardness of the nucleus, ocular tension, cornea astigmatism and the cornea central endothelium account etc. in the statistics analysis.The eye sight, slit lamp, ocular tension, change of corneal curvature and the cornea central endothelium of the patients operated were checked in the 1d, 3d, 7d, 1month and 3month.The best naked vision≥0. 5 was 74 cases (80. 43%) in the water flushing group and 51 cases (67. 11%) in the routine group. There shows significant differences (P<0. 05) between the two groups.All the corneas were clear and bright in the 1d after surgery, 2 cases in the routine group were slight corneal edema without obvious Descemet' s reductus. Every ten minutes of half day, eyedrop application (50% GLU solid ) was given into partly of eye, the next day, corneal edema disappeared and the corneal recovered clearly and brightly.There was no Inflammatory reaction of anterior chamber in both of the two groups after surgery, the pupil keep circle and sensitive to light reflex All ocular tension keep normal. Incisions closed well after surgery of the two groups.The cornea astigmatism of water flushing group compared with routine group after surgery, shows no significant differences( P>0. 05). The cornea astigmatism of the water flushing groups shows no significant differences( P>0. 05), but The cornea astigmatism of the routine group shows no significant differences( P<0. 01) .The cornea central endothelium account before surgery compare with after surgery within 3 months, the cornea central endothelium account of water flushing group compared with routine group after surgery, shows significant differences(P<0. 05)But the cornea central endothelium account of the routine group, water flushing group shows no significant differences(P<0. 01) between before surgery and after. After surgery, the loss rate of cornea central endothelium account: water flushing group (9.02±0.69) %, routine group (13.21±1.13) %, shows significant differences( P<0. 05) between two groups.Firstly, the group of water flushing method with small incision nonphacoemulsification cataract surgery and implantation of intraocular lens is better than the group of small incision nonphacoemulsification surgery with respect to eyesight, cornea astigmatism and the cornea central endothelium account. Secondly, water flushing method has the same clinical effect with phacoemulsification cataract surgery, and the lose percentage of the cornea endothelium cells is lower. This method deserves to be popularized. Especily, the area with many eyes surgery, lack of condition to help people to recover the Bright Sight Project and incomplete surgical apparatusbasic leve hospital.
