The characteristic of AIDS is that human immunodeficiency virus type1(HIV-1) infect the human body,causes CD4+ lymphocyte counts reducte and the immune function is affected.Infection with HIV-1 causes an immune activation with some expression of immunological makers.The earliest markers used included symptoms and CD4+ lymphocyte counts.Unfortunately,CD4+ lymphocyte counts are not very discriminating of prognosis,highly variable over time,and considerable overlap in CD4+ cell counts among the clinically defined stages of HIV infection has been found. There is no difference for the CD4+ lymphocyte counts between progressors and nonprogressors in asymptomatic individuals.However,the immune level between them is markedly different in fact.In addition,CD4+ lymphocyte counts have been found variable with age,smoking,etc.In addition,HIV RNA copies is the common effect of viral replication and immune function.So it is an important marker for the development of HIV infection.In conclusion,accurate indices for monitoring the disease process are required for clinical and therapeutic management of asymptomatic HIV-infected persons with Chinese medicine.Immune activation markers including TNF-α,sTNFRs,IL-2,IL-6, IL-7,IL-10,IL-12,IL-16 are reviewed here.Soluble tumor necrosis factor receptors(sTNFRs),including sTNFR-Ⅰand sTNFR-Ⅱ,correlate with clinical progression of HIV infection,which may play an important pathogenic role in HIV infection.The correlation among sTNFR-Ⅰ,CD4+ lymphocyte counts and HIV RNA copies (viral loads) in PBMC were analyzed,and immunological changes of HIV infected individuals with disease development was discussed here.The results indicate negative correlation was observed between sTNFR-Ⅰand CD4+ lymphocyte counts(r=-0.557,P<0.05).Positive correlation was observed between sTNFR-Ⅰand HIV viral load(r=0.388).In addition,the results indicated the level of sTNFR-Ⅰ(CD4+ lymphocyte counts<200/mm3) is higher significantly than the same(CD4+ lymphocyte counts>200/mm3)(P<0.01).The results above suggest that sTNFR-Ⅰis correlated with development of HIV infection,and it maybe useful to evaluate level of sTNFR-Ⅰ,CD4+ lymphocyte counts and HIV viral load of HIV infection patients with Chinese Medicine. With rapid development of molecular immunology,more and more immunological makers correlated with HIV were found.It is useful for the evaluations of changement of immune system in patients with HIV/AIDS.Immune-based intervention is regression of Chinese medicines,better indicators, including sTNFR-Ⅰ,for monitoring the clinical progression are required for therapeutic management of HIV-infection with Chinese medicine.
