Objective①To objectively evaluate the upper limb movement function of stroke patients according to sEMG signal signature analysis.②To observe the effect on the functional reconstruction of shoulder and elbow after stroke by stimulating stellate ganglion,brachial nerves,acupoints on three yang channels of hand and muscular movement points on shoulder and elbow with pulse modulation medium frequency electric.Optimize the treatment.Methods①Apply sEMG collection system to collect the sEMG signals of agonistic muscle and assistant muscle when 30 cases stroke patients move the shoulders and elbows.②Extract characteristic value of sEMG.Compare the sEMG of patients' affected side and healthy side when moving shoulders and elbows.③Divide the 30 cases into three groups randomly.Stimulate stellate ganglion,brachial nerves,acupoints on three yang channels of hand and muscular movement points with pulse modulation medium frequency electric. Treatment:once per day,six times a week for 2 months.④Evaluate the clinical effect of every group according to sEMG and FMA,ADL Barthel index scale pre and post treatmentResults①Surface electromyogram signal has no change if muscle doesn't have active contraction in peaceful state.②When stroke patients move their shoulders and elbows,characteristic value of sEMG such as iEMG,AVE,MAX of agonistic muscle and assistant muscle of healthy side and affected side are different and the difference is significant(P<0.01)③When stroke patients move triangular muscle,characteristic value of sEMG such as iEMG,AVE have significant lifference P<0.05,MAX has no significant difference P>0.05 ④Compared to pre treatment characteristic value of sEMG such as iEMG,AVE,MAX have significant difference(P<0.01)after being treated by pulse modulation medium frequency electric.⑤After being treated by pulse modulation medium frequency electric, sEMG evaluation and muscle functional reconstruction of stroke patients among the three groups are different.Effect on functional reconstruction of supraspinatus,pectoralis major of group A is superior to that of the other two groups.Effect on functional reconstruction of triangular muscle,triceps brachii and anconeus of group B is superior to that of the other two groups.⑥After being treated by pulse modulation medium frequency electric, the scale evaluation of stroke patients of each groups have significant difference(P<0.05)⑦After being treated by pulse modulation medium frequency electric, the scale evaluation of stroke patients among the three groups have significant difference(P<0.05),group B and group C are superior to group A.Conclusions①Stroke patients' sEMG have a significant difference between the healthy sides and affected sides,sEMG can objectively and sensitively reflect the muscle functional and active state of sroke patients.②sEMG and scale evaluation complement to each other.Objectively reflect stroke patients' state.③Stimulate stellate ganglion,brachial nerves with pulse modulation medium frequency electric has significant effect on functional reconstruction of shoulders joints.④Stimulate stellate ganglion,brachial nerves,acupoints on three yang channels of hand with pulse modulation medium frequency electric has better effect on functional reconstruction of shoulders and elbows joints.
