Objective : To investigate the risk factors of early postoperative complications in patients undergoing pancreatoduodenectomy and to analyse the causes of postoperative complications of PD and study measures for prevention and treatment of the complications.Methods : Clinical data of 68 patients undergoing pancreato- duodenectomy in our hospital from January 2002 to December 2007 were collected and analyzed retrospectively.Ten clinical factors were recruited for the study in relation to surgical complications.Results : Postoperative complications occurred in 24 patients (35.3%).Of these 24 patients,4(5.9 of the total)died.Logistic regression analysis revealed that no billiary stent (OR=12.841), preoperative total serllm bilirubin level over 171.1umol/L (OR=8.990) , end-to-end pancreaticojejunostomy (OR=8.542),no pancreatic stent (OR=8.522),intraoperative blood transfusion over 1000 ml (OR=8.385) were important factors for early complications.Conclusions : The main complications after PD were pancreatic leakage,bile duct leakage and hemorhage.Pancreatic stent ,billiary stent, end-to-side mucosa-to-mucosa pancreaticojejunostomy and good surgical expertise can decrease early morbidity rate. |
