Study On The FGIDs And Their Symptom Overlap In The Gastrointestinal Outpatient Department

Objective:FGIDs(functional gastrointestinal disorders)was a aggregate of subgroup based on the symptoms defined by Rome criteria,and without structural or biochemical abnormality.The epidemiological data suggested that some symptoms of FGIDs were overlapped.Although the prevalence of the FGIDs was stable over time,the turnover in symptom status was high.The aim of the study was to discuss the proportion of FGIDs,distribution of the subgroups and the status of the overlap of gastrointestional symptoms in gastrointestinal endoscopic center of the General Hospital of Tianjin Medical University.Methods:According to RomeⅢcriteria,categorizing the patients into FGIDs and OGIDs,then analyzing the following symptoms and status of overlap,including FDy, Globus,BD,FD,IBS,FB,FDi and FC.Results:1.3552 patients were entered into the study,2255(63.5%)were diagnosed as FGIDs and 1297(36.5%)were diagnosed as OGIDs.There was no significant statistical difference in body mass index between FGIDs patients and OGIDs patients (P>0.05).The age,gender,career and education level were significantly different(P<0.05).The peak of the age was 40~60 years in FGIDS and 50~70 years in OGIDs. Females,higher education ones and brainworkers in FGIDs were more than those in OGIDs.2.Among 2255 patients of FGIDs,61 patients(2.7%)had FDy,101 patients (4.5%)had Globus,1943 patients(86.2%)had FD,45 patients(2.0%)had BD,279 patients(12.4%)had IBS,48 patients(2.1%)had FB,40 patients(1.8%)had FDi and 252 patients(11.2%)had FC.There were significant statistical differences in gender and age among subgroups(P<0.05).3.Among 2255 patients of FGIDs,1704 patients(75.6%)had a single symptom, 506 patients(22.4%)had two-way symptom combinations,42 patients(1.9%)had three-way symptom combinations,3 patients(0.1%)had four-way symptom combinations.There were 25 forms of overlap in FD,14 forms of overlap in FC,13 forms of overlap in Globus,12 forms of overlap in BD,10 forms of overlap in IBS and FDy,9 forms of overlap in FB,there was only one form of overlap in FDi.No significant statistical differences were found in body mass index,gender, career and disease duration between patients with and without overlapping symptoms (P>0.05).There were significant statistical differences in age and education level(P<0.05).There was no significant statistical difference in the severity of the seven common symptoms of dyspepsia between FD patients with and without overlapping symptoms(P<0.05).Conclusions:1.About 2/3 patients were diagnosed as FGIDs,1/3 patients were OGIDs.The peak of the age was 40~60 years in FGIDs and 50~70 years in OGIDs.Females, higher education ones and brainworkers in FGIDs were more than those in OGIDs.2.In the subgroups of FGIDs,the ratio of patients with FD was the highest,the following were patients with IBS and FC.In different subgroups,FDi was more prevalent in male patients,the other subgroups were more prevalent in female patients;FB and FDi were more prevalent in old patients,the other subgroups were more prevalent in midlife patients.3.Symptom complex overlap was common among FGIDs,about 1/3 patients had two- symptoms complexes or three- symptoms complexes.In one kind of FGIDs, it represented many forms of overlap.Studies indicated that overlap of symptoms might be based on the same pathophysiological mechanism.The overlap of symptoms suggested that FGIDs patients had the dysfunction in both upper and lower alimentary tract at the same time.So we should pay attention to the diagnosis and do our best to make it integrity.Otherwise,the characteristics of some disorders can not be reflected comprehensively by the classified and diagnostic criteria at present.
