| 0bjective To analyze the feasibility of treatment of bone tumor around the knee joint wiht a prosthesis-devitalized bone composite.Methods 11 cases receive the treatment of bone tumor around the knee joint with a prosthesis-devitalized bone composite. There are 8 males, 3 females. The age is from 12 years to 53 years, with an average of 24.9 years. There are 8 cases with osteosarcoma, 1 case with fasciculated sarcoma, 1 case with malignant fibrous histiocytoma, 1 case with recurrence of giant cell tumor of bone. The diseased region at the distal femur are 8 cases, at the proximal tibia are 3 cases. According to Enneking's classification, 10 case with malignant bone tumor were classified at IIB. The bone tumor were widely resected in all case. The resected bone were debrided off gross tumor tissues, and treated with boiling for 30 minutes, then the devitalized bone and prosthesis were maked up a composite. Reconstruction of the affected knee joint after major joint removal was done by the prosthesis-devitalized bone composite.After the operation, the affected limb were wrapped with pressure bandages for one month.Results 11 patients were followed up from 6 to 20 months with an average of 12.9 months. 1 cases died within 11 months following operation because of the metastatic cancers of lung;1 case has common peroneal nerve injury;1 case has cutaneous necrosis of the ankle because the pressure bandages wrapped the affected limb too tight; and the others were alive without deseases . No cases recurrence postoperatively ; All patients had no dislocation,1oosening or breakage of prosthesis. X—rays showed the time of bone healing between devitalized bone and host femur or tibial was an average of 8.2 months. 11 cases had good active ension—flexion degrees of an average of 85.4°(range,75°to 105°).The postoperative average IS0LS scale score of 11 cases by Enneking's system was 25.4 point(range, 22 to 28).Conclusion The clinical result demonstrate composite of devitalized bone and prosthesis replacement has the advantages that the affected limbs of patients can have good function with few complication. The recurrence rate is lower too. It is one of safe and excellent methods of reconstruction of the affected knee joint after the tumor was widely resected. |
