Objective:To assess the angiogenesis correlation among color Doppler flow imaging(CDFI) Indexes,vascular endothelial growth factor (VEGF) and the microvascular density (MVD)in thyroid tumors,and to evaluate their clinical value.MaterialandMethod: Thirty-eigth thyroid tumors were examined preoperatively with CDFI, According to previous literatures, CDFI flow patterns will be divided into 0~III patterns.Doppler blood flow signal was detected, vascular resistance index(RI)was Measured three times and choosed the average. Tumors'VEGF protain and microvessels were stained immunohistochemically by using anti-VEGF and anti-CD34 respectively in surgical speciments after operation. There are 20 cases thyroid cancers, and 18 cases thyroid adenomas,all patients were not treated preoperatively. The results was tested by the t-test, rank sum test and correlation analysis.Results: 1. The comparison of CDFI and hemodynamic parameters (Vmax,RI )in Thyroid tumors:(1) Category III in the blood flow was dominated in thyroid cancers (10 cases), ClassII of the five cases,I rank of the three cases, Class I was dominated in thyroid adenomas(11 cases), ClassII of the four cases, class 0 of the three cases. The CDFI flow classification was statistically significant between thyroid cancers and thyroid adenomas (P<0.05).⑵. The spectrum of systolic blood flow appeared the characteristics of high- RI (0.61±0.25) in thyroid cancers, the spectrum of systolic blood flow appeared low RI (0.43±0.22)in thyroid adenomas, the RI wasstatistically significant in the two groups(P<0.05). the speed was not statistically significant in the two groups(P>0.05).2. The immunohistochemical staining results in thyroid tumors:(1)VEGF-positive staining appeared yellow to brown granules in the cytoplasm, and the degree and size of positive staining cells was different, the expression of VEGF was statistically significant between benign and malignant tumors(P<0.01).(2)CD34-positive staining appeared brown endothelial cells, CD34 immunohistochemical staining was all positive in thyroid tissue biopsies, MVD was counted for 67.47±23.83 per 200 times View in thyroid cancers,and MVD was 35.28±26.66 in thyroid adenomas,the count for MVD was statistically significant between benign and malignant tumors(P<0.01).(3)The correlation Analysis between the expression of VEGF and the count for MVD in thyroid tumor: the correlation coefficient was 0.697, there was correlation (P <0.01), the expression of VEGF was positively correlated with the count for MVD statistically significant in thyroid tumors.3. The correlation Analysis between the blood flow classification and the count for MVD,the expression of VEGF in thyroid tumor: the correlation coefficient was respectively 0.722,0.718, there was correlation (P <0.01), the blood flow classification was positively correlated with the count for MVD statistically significant in thyroid tumors.4. The correlation Analysis between the count for MVD and RI in thyroid tumors: the correlation coefficient was 0.712, there was correlation (P <0.01), the count for MVD was positively correlated with RI statistically significant in thyroid tumors.Conclusion: CDFI indexes, the degree of VEGF and MVD may provide important information for diagnosing and evaluating thyroid tumors from different viewpoint.
