Objective:To investigate the histogenesis and clinicopathologic feature of sclerosing hemangioma of lung (SHL) and to study the biological behavior of SHL.Methods: Based on the microscopical features, 39 cases of SHL were classified and stained by HE. EMA, MCK, TTF-1, Vimentin, Syn, CgA, CD34, factorⅧ, S-100, CD68, Calratinin, CD95, CD44v6, P53, Ki-67 of patients were examined immunohistochemistry. The statistical analysis was performed using SPSS 11.5 statistical package and numeration data were analyzed withχ2 test and rank test.Results:①SHL has a characteristic architecture composed of round cells and surfacecubic cells. It has a variety of tissular structure, mainly expression with four regions that contain solid regions, papillary regions, angiomatoid regions and sclerosing regions. Most cases composed by 2 to 4 tissular structures, and papillary structure is very common.②Coexpression of EMA, Vimentin and TTF-1 in both cuboidal and polygonal cells and un-expression of CD34,factorⅧ, CD68, Calratinin were found. Both cells were diffusely immunostained with S-100, CgA, and Syn. MCK expressed in surfacebubic cell and diffused expressed in round cell. There were no significant difference of other indexes between these two cells.③CD44v6 expressed in surfacebubic cells (94.87%), there was significant difference of expression of CD44v6 between two kinds of cells (P<0.05). The expression of CD44v6 and Ki-67 were higher in SHL than that in normal tissue of lung (94.87% vs 10.00% , P <0.05; 46.15% vs 15.00% , P <0.05).The expression of CD95 was lower in SHL than in normal tissue of lung (5.13% vs 85.00% , P<0.05).④Clinical feature: in this study, most patients were female, tumor was always found in right lung and inferior lobe, single,and symptoms were rare.Conclusions: SHL may origin from the primitive respiratory epithelial. The expression of CD95, CD44v6 and Ki-67 has significant difference in SHL and in normal tissue of lung. The SHL has malignant potentiality.
