Prevalence Of Autoimmune Thyroiditis Of The Diabetic In-patients

Objectives: To investigate the prevalence of autoimmune thyroiditis(AIT) of the diabetic in-patients.Methods: The frequency of thyroid peroxidase antibody(TPO-Ab)was determined in 471 subjects,including 377 type 2 diabetes mellitus(T2DM), 19 classic type 1 diabeties (T1DM),11 latent autoimmune diabetes in adults(LADA)and 64"untype"diabetics(NTDM).Results: (1) TPO-Ab existed less frequently in T2DM (14.59%,55/377) than other three groups[in T1DM patients (42.11%,8/19;P>0.05),in NTDM patients(31.25% , 20/64 ; P>0.05),in LADA patients(63.64% , 7/11 ;P<0.05)];comparing with LADA and T2DM groups,there was significant difference;while comparing with T1DM,NTDM and T2DM groups,there was no significant difference. (2)There was no significant difference in LADA and NTDM groups in age,course of disease and BMI. (3)Besides the rate of antoimmune thyroid diseases(AITD) and sexual has significant difference(P<0.05),there was no significant difference in the positive and negative of TPO-Ab in T2DM groups in the clinical feature of age,course of disease,history disease of antoimmune disease,BMI,the rate of insulin treatment,C peptide of not eating,C peptide of 2 hours after eating,the unusual rate of antithyroid glububin antibody(s-TSH) and subclinical hypothyroidism(P>0.05). (4)16.4% (9/55) of T2DM patients with thyroid autoimmunity had thyroid dysfunction as compared with 8.69% (28/322) in the group without thyroid antibodies.Conclusions:(1) The positive of TPO-Ab in T2DM patients was 14.6%. (2)The rate of thyroid dysfunction in the positive of TPO-Ab in T2DM patients was clearly higher than that of the negative. (3)The diabete patients that of the intitial stage of flaring up of disease,the beginning of the ketosis,the fair function of insulin,but of being inclined to LADA and being all negative to antoantibodies of insulinβ-cell were not divided into groups temporarily.The positive of TPO-Ab in untype diabetics was higher than that of T2DM,but lower than that of T1DM and LADA. (4)The positive rate of TPO-Ab in clinical diagnosis LADA patients was the highest in the four groups.It pointed that the illness mechanisms in LADA and AIT have the common characteristics. (5)From T2DM, NTDM,and classic T1DM,to LADA,the rate of the positive of TPO-Ab has the tendency of being increased continuously .
