The Clinical Study To The Multisection Spiral CT Perfusion On Patients Of Acute Pancreatitis

Objective To quantitative analysis and assess the blood perfusion characteristic of normal pancreas by Multisection dynamic CT. To investigate the perfusion parameters's change according to age,sex and difference locations of normal pancreas.Methods 30 cases with normal pancreas were examined for pancreatic perfusion and was divided into three sub-group according to age. The multisection dynamic CT perfusion series was performed for pancreatic perfusion on a multisection CT scanner(Siemens somatom Sensation 64),120kV, 100mA.Contrast injection was done by using 50 ml nonionic contrast agent(300 mgI/ml ),at a flow rate of 5ml/s with a power injector,and 6 seconds delay. The data acquisition lasted for 30 seconds.These datas were processed on a SIEMENS workstation using PCT software package ,and the mean BF, BV, TTP and PS were measured and statistically analyzed in 30 cases with normal pancreas.Result The study showed that all perfusion parameters didn't influenced by age,sex and difference locations of pancreas(P>0.05).The mean BF, BV, TTP and PS in normal pancreas tissue were 133.55±25.70(ml/100ml/min),193.78±27.01(mL/L),142.41±13.95(0.1S),128.72±34.53(0.5 ml/100ml/min),respectively. The perfusion parameters in three sub-group according to age were not statistically significant(P>0.05). The mean BF, BV, TTP and PS in pancreas of male were 127.59±22.90(ml/100ml/min),188.72±25.19(mL/L),142.91±13.17(0.1S),133.30±42.86(0.5ml/100ml/min), respectively and those parameters of female were 142.48±28.04(ml/100ml/min),201.37±28.94(mL/L),141.66±15.63(0.1S),121.85±14.80(0.5ml/100ml/min), respectively. The perfusion parameters in male and female were not statistically significant(P>0.05). The perfusion parameters in difference locations of pancreas were also not statistically significant(P>0.05). Conclusions MSCT perfusion imaging may reflect the features of normal pancreatic perfusion. All of perfusion parameters didn't influenced by age,sex and difference positions of pancreas. Further more,detected the changing of the perfusion parameters in the cases of AP, and the relation between the perfusion parameters and severity. To assess the value of the perfusion parameters as severity indicators in acute pancreatitis.Methods 82 cases were examined for pancreatic perfusion from august in 2006 to January in 2008.The multisection dynamic CT perfusion series was performed by a multisection CT scanner(Siemens somatom Sensation 64),120kV, 100mA after 48-72h admission and 77 patients completed dynamic CT perfusion successifully.Contrast injection was done by using 50 ml nonionic contrast agent(300 mgI/ml ),at a flow rate of 5ml/s with a power injector, 6 seconds delay,and data acquisition lasted for 30 seconds.These datas were processed on a SIEMENS workstation using PCT software package ,and the mean BF,BV,TTP,PS were measured and statistically analyzed in 77 patients with pancreatitis.Result The mean BF, BV, TTP and PS in AP were 110.57±60.04 (ml/ 100ml/min),156.68±65.11(mL/L),146.58±29.46(0.1S),110.73±62.66(0.5ml/100ml/min), respectively.The mean BF, BV between AP and normal pancreas were statistically significant(P<0.05). However,the mean TTP,PS between two groups was not statistically significant(P>0.05). The mean BF, BV, TTP and PS in SAP were 77.52±21.94(ml/100ml/min),114.08±26.39(mL/L),153.08±26.25(0.1S),110.53±51.87(0.5ml/100ml/min), respectively. The mean BF,BV,TTP and PS in MAP were 124.65±65.52(ml/100ml/min),174.83±68.31(mL/L),143.82±30.53(0.1S),110.77±67.16(0.5ml/100ml/min), respectively.Comparing to MAP and normal pancreas,the BF,BV were significant decreased in SAP (P<0.05). The BF,BV were not statistically significant between MSP and normal pancreas. The hospitalization days were delayed in SAP(20.91±8.40d) contrast with MAP(11.51±8.46d, P<0.05)). But The time of abdominal pain relieved were not statistically significant between SAP and MAP(P>0.05). It was found that BV,BF had a correlation with APACHEⅡcriteria,Ranson criteria,CRP,CTSI,hospitalization days and local complications.Conclusions MSCT perfusion imaging may reflect the features of pancreatic perfusion in acute pancreatitis. The BF and BV were markedly decreased in AP and correlated with severity. The BF and BV can predicted severity of acute pancreatitis patients.
