Study On Left Ventricular Regional Systolic Function Of Coronary Artery Disease With Regional Wall Motion Abnormality By Real-Time Three-Dimensional Echocardiography

Objective To evaluate the feasibility and veracity of real-time three-dimensional echocardiography (RT-3DE) in quantifying left ventricular regional systolic function of the patients with coronary artery disease who have regional wall motion abnormality (RWMA).Methods 30 healthy volunteers (normal group) and 25 patients with coronary artery disease who have regional wall motion abnormality (experimental group) were examined by RT-3DE and RT-3DE images were obtained. Quantitive analysis of those RT-3DE images was then performed by software, and global and 17 regional LV volume-time curves were drawn. Then, regional end-diastolic volume (REDV), regional end-systolic volume (RESV), regional stroke volume (RSV) and regional ejection fraction (REF) were extracted from the curves, and the comparison and analysis on the values of the two groups were carried out.Results The features of normal group were as follows: the REDV, RESV and RSV values increased from apex to base; the REF values decreased from apex to base; septal REF values, when divided into anterior and inferior regions, were significantly reduced, compared with those in the free-wall regions (inferior, lateral and anterior wall). The features of experimental group were as follows: the REDV and RESV values were significantly higher than those of normal group respectively (P<0.05);the RSV values of significantly hypokinetic segments (anterior, anteroseptal and inferior wall) were lower than those of normal group; the RSV values of no obvious RWMA (anterolateral and inferolateral wall) were higher than those of normal group; the REF values were significantly lower than those of normal group (P< 0.05), and REF values of significantly hypokinetic segments decreased more significantly than other segments (P< 0.01).Conclusions (1) It is highly feasible and credible to quantitively evaluate the regional left ventricular myocardial contraction of coronary artery disease with regional wall motion abnormality by RT-3DE. (2) Normal regional left ventricular myocardial systolic has heterogeneity. (3) The left ventricular regional systolic function of hypokinetic segments of the patients with coronary artery disease was significantly decreased.
