OBJECTIVE:To investigate the etiology,and therapies of primary adrenocortical hypofunction(Addison' s disease).METHODS:To analyse the clinical data of 26 patients with primary adrenocortical hypofunction admitted at The NO.1 Affiliated Hospital of Guangxi Medical University from December 2002 to July 2007.Here it refers to the clinical characteristics,laboratory and instrument examination,and therapies.RESULT:The patients' age ranged from 14 to 66.26 cases Addison' s disease.26 Addison' s disease etiologically included 7 adrenal tuberculosis,14 idiopathic,4 patients had a history of adrenal cortex adenoma resection operation,1 remained unknown.The typical history of malaise, fatigue,anorexia,weightloss,dizziness,orthostatic hypotension,vomiting,abdominal distention,abdominal pain and darkening of the skin.Patients might crave salt and develop unusual food preferences,such as drinking the brine surrounding pickles.The laboratory examinations included: diminished,secretion of cortisone,hypopotassemia, hyponatremia,hypoglycemia,hyperkalemia and anemia. Abnormity could be found in adrenal CT.Treatment of adrenal insufficiency involved physiologic replacement of the deficient hormones.For chronic therapy,oral hydrocortisone provided glucocorticoid replacement that could be easily titrated and given as divided doses during the day to grossly recapitulate the normal diurnal pattern of cortisol secretion.Patients should add the high dose of Cortisol temporarily when they are at stress.
