| The titanium-polymerization porcelain repair technology is already widely applied in the crown bridge repair, but still many basic studies and clinical trials needs to be done. Researchers have carried out a series of work such as"Study of the shear bond strength of titanium or its alloys with solidex veneering resin"and"Study of titanium and hard composite resin bonding properties", and have achieved many sound experimental results. The bonding strength of titanium-polymerization porcelain repairs body has been greatly improved and basically met the criteria for clinical application. However, compared with the bonding strength of titanium-porcelain repairs body, its bonding strength is relatively low.Based on the previous studies, exploratory study was carried out aiming to finding better methods to enhance the bonding strength of titanium-polymerization porcelain, namely through after the pure titanium surface deposited with the TiN thin film , then roasts with ceramage polymerization porcelain. After accelerating aging of the polymerization porcelain material through the toothbrush cleaning experiment, changes of its surface contours, gloss and color were observed, providing the references for the clinical application. Meanwhile, the clinical experiments of the titanium- polymerization porcelain crown repair were carried out and follow-ups were made to observe the therapeutic efficacies.Experimental results1. Results of the cutting experiment.Shearing strengths of both two groups exceeded the ISO9693:1999 standard. The TiN coating group's titanium- polymerization porcelain bonding strength significantly enhanced compared to that of the conventional treatment group. The scanning electron microscope demonstrated: In the coating group titanium-polymerization porcelain fracture, the polymerization porcelain residuals were more than in the conventional treatment group; In the coating group's titanium-polymerization porcelain contact surface, approximate 0.3μm TiN membrane existed, and titanium and polymerization porcelain can be closely integrated; But in the conventional treatment group, crevice existed between the titanium-polymerization porcelain, which gave explanation to the bad wetting quality of polymerization porcelain toward titanium and probably accounts for its low bonding strength.2. Before or after the brushing experiment, all the testing indexes of ceramage polymerization porcelain were better than that of solidex polymerization porcelain, indicating that the polymerization porcelain system of ceramage surpass the former polymerization porcelain system in the wear resistance and the color stability. In the ceramage group, the test sample skin hardness, roughness increase in value, index of reflection drop value, brilliance change value and the chromatic aberration value differed greatly in the wool tooth brush assembly and the bristle tooth brush assembly. In the solidex group, differences were also significant in the soft toothbrush group and hard toothbrush group. Electron microscopy demonstrated that the brush marks of bristle tooth brush assembly is more obvious than that of the wool tooth brush assembly, indicating that the bristle toothbrush affected more to the surface contours, gloss and color of the polymerization porcelain than the wool toothbrush.3. One year follow-ups were carried out in 30 cases of dental defects or teeth missing patients after titanium-polymerization porcelain crown repair. Evaluations were made according to the color coordination, marginal fitness, coloring, surface patterns, loss, gum tissue, and other indicators. Satisfaction degrees were also included in the survey. Finally, four patients lost visits in the last reexamination, and the remaining patients received regular reexamination in the follow-up period. All the indexes of the repaired bodies achieved satisfactory repair effects. All patients after repairing expressed satisfaction or basic satisfaction and none case of allergy occurred.Conclusion1. Bonding strengths of both the two groups have met the standards for clinical application. TiN film processing to the surface of titanium may enhance the bonding strength of titanium and the polymerization porcelain effectively.2. The polymerization porcelain system of ceramage surpasses the former polymerization porcelain systems in the wear resistance and the color stability. It not only enhances repairs body's service life, but may also enhance repairs body's use quality. However, toothbrush's property has great influence on the surface texture and the gloss of both two polymerization porcelains. Therefore, in the clinical use of polymerization porcelain repair, the patient should be recommended to use the wool toothbrush to clean the teeth in order to lengthen the repairs body's usage period.3. Through the clinical observation, the author observed that in a short time (1year-1.5year), the titanium-polymerization porcelain crown repair can efficiently restore the tooth body's shape and function with satisfactory repair effect. Its sound wear resistance and extremely good biological compatibility guarantees the health of clutch tooth and the dental capsule. In conclusion, it is one kind of ideal repair body. |
