Study On Erythrocyte Immune Adhesion Function And Expression Of CD58 And Effects Of Lumbrukinase In Patients With Chronic Renal Failure

Objective: To investigate the dynamic change and clinical significance of red-cell adhesion function and expression of CD58 molecules (RBC lymphocyte function- associated antigen 3, LFA-3) on erythrocyte membrane and it' s relationship between these parameters and creatinine clearance in patients with chronic renal failure . And effects of lumbrokinase to the red-blood-cell adhesion function and the expression of CD58. Methods: Thirty-four patients with chronic renal failure were selected and taken lumbrukinase to enteric-coated capsules in the study. Serum creatinine and its conversion rate of creatinine clearance were measured before and after treatment. Red cell C3b Receptor Rosette(RBC-C3bRR), Red cell Immune Complex Rosette (RBC-ICRR), the promotion and inhibition rate of Red cell C3b Receptor Rosette and the expression of CD58 were also measured before and after treatment. The above indicators were measured in normal adult 35 cases as control group. Results: The RBC-C3bRR, promotion rate and CD58 , including before and after taking lumbrukinase, were significantly lower (P<0.05) than normal control group in patients with CRF; RBC-ICRR and inhibition rate were higher (P<0.05). Furthermore,the RBC-C3bRR and promotion rate after taking lumbrukinase were significantly higher (P<0.05) than them before taking lumbrukinase, RBC-ICRR and inhibition rate were lower (P<0.05). But the difference of CD58 was not statistically significant (Pï¹¥0.05). The RBC-C3bRR, promotion rate, CD58 and conversion rate of creatinine clearance were positive correlation (P<0.05); inhibition rate and conversion rate of creatinine clearance were certain negative correlation (P < 0.05). There was low correlation between RBC-ICRR and creatinine clearance. Patients with CRF were divided into three groups according to etiology: diabetic nephropathy, hypertensive renal sclerosis and nephritis group. The RBC-C3bRR and promotion rate in diabetic nephropathy and hypertensive renal sclerosis were significantly higher (P<0.05) than them in nephritis group;RBC-ICRR and inhibition rate were lower (P<0.05). CD58 in the three groups were showed no significant difference (Pï¹¥0.05). Conclusion: Red-cell immune function and expression of CD58 were decreased in patients with the CRF, and they has some relevance to the level of creatinine clearance. Lumbrukinase can, to a certain extent, improve the immune function of erythrocyte in patients with CRF, but Lumbrukinase can not obviously impact on the expression of CD58.
