Objective:To study the expression of HMGB1 in the systemic lupus erythematosus(SLE)and SLE secondary infection,to find a new method to diagnosis and treatment of the SLE patients secondary infection.Methods:From September 1,2007 to December 31,2007.80 volunteers from the Xiangya hospital in-patients and out-patients were randomly selected,including 20 with active SLE and 20 with stable SLE,20 SLE patients with lung infection,and the rest were healthy.Ficoll density gradient centrifugation was used to separate monocytes from peripheral blood.Monocytes were treated by 500ng/ml LPS or 0ng/ml LPS.The monocytes were cultured in incubator with 37℃and 5%CO2 for 24 hours.Then By ELISA method all volunteers were detected,In addition, Western blot was applied for detection of HMGB 1 in culture medium. The data was analyzed via SPSS 15.0.Results:(1)There was no statistically significant difference of the concentrations of HMGB1 in culture medium of peripheral blood monocytes from stable SLE patients compared with healthy controls (P>0.05).It was higher on the concentrations of HMGB1 in culture medium from active SLE,compared with stable SLE patients and healthy controls.It was higher on the concentrations of HMGB1 in culture medium from active SLE with pulmonary infection,compared with active SLE.(P<0.05).(2)There was no statistically significant difference of the concentrations of HMGB1 in culture medium of peripheral blood monocytes after stimulated with LPS from stable SLE patients compared with healthy controls(P>0.05),It was higher on the concentrations of HMGB1 in culture medium from active SLE,compared with stable SLE patients and healthy controls.It was higher on the concentrations of HMGB1 in culture medium from active SLE with pulmonary infection, compared with active SLE.(P<0.05).(3)There was statistically significant difference of the concentrations of HMGB1 in culture medium of peripheral blood monocytes after stimulated with LPS in stable SLE patients and healthy controls(P<0.05). There was no statistically significant difference of the concentrations of HMGB1 in culture medium of peripheral blood monocytes after stimulated with LPS in active SLE patients and active SLE with pulmonary infection(P>0.05).(4)The expression of HMGB1 in the serum from active SLE(75%)and active SLE with pulmonary infection(85%)can be detected by Western methods,it was significantly higher in serum from active SLE with pulmonary infection(P<0.05),compared with active SLE.(5)The concentration of HMGB1 in culture medium of SLE was positively correlated with SLEDAI(rs was respectively 0.5415,,P<0.05).Conclusions:(1)The concentration of HMGB 1 in culture medium from active SLE patients increased,and the increased level of HMGB1 related activities of disease and the higher concentration of HMGB1 from SLE with pulmonary infection.(2)It was significantly higher on the expression of HMGB1 in serum from active SLE with pulmonary infection,compared with other SLE.
