The Investigation On Drinking Behavior Of The Male HBV Carrier In XiangYa Hospital Outpatient Service

Objective: (1) To understand the epidemiological characteristics of the drinking behavior of the male hepatitis B virus (HBV) carriers. (2 ) To study the relation between the drinking behavior of the male HBV carriers and general state of the cases, liver function, emotional state ( anxiety and depression ). ( 3 ) To provide the scientific evidence for intervening the drinking behavior of the male HBV carriers.Methods: 935 cases of outpatient male HBV carriers were interviewed by researcher with structural questionnaires including drinking general state questionnaire, The Alcohol Use Disorders Identification Test (AUDIT), Self-Rating Anxiety Scale(SAS), Beck Depression Inventory (BDI). Statistical analysis were carried out with SPSS15.0 statistical package. The statistical methods included descriptive statistics, chi square test, t-test, ANOVA, Pearson analysis and polynary stepwise regression analysis.Results: (1) The month drinking rate and the year drinking rate of the outpatient's male HBV carrier were 35.0% and 58.2%, respectively. ( 2 ) The highest group of the year drinking rate and the month drinking rate was at the age of 20~29 years old, and the rate reached 64.5% and 39.2%, respectively. According to the educational background, the cases with bachelor degree or master degree or doctor degree had the highest rate, the two rates were 75.1% and 46.7%, respectively. In the profession aspect, the cases with officer and professional staff had the highest rate, the two rates reached 72.0% and 50.2% respectively. The two rates of the cases who could earn no less than 2000 yuan per month were 65.2% and 42.4%, respectively. ( 3 ) The average every time capacity for liquor and the per capita year capacity for liquor of the drinker in the male HBV carrier were 39.81±51.58 ml and 1.68±6.44L absolute ethanol. And beer was the first choice. ( 4 ) The initial drinking age in the male HBV carrier was 17.3±5.2 years old. Among the cases, the age of 63.5% drinkers was no more than 18 years old. ( 5 ) The investigation showed that heavy drinking was 2.6%, frequent drinking of four times per week was 6.6%, year fast drinking was 31.5%, 17.9% drinkers were drinking high degree alcohol and 39.4% drinkers were drinking mixed wine, 9.1% drinkers were only drinking and 3.8% drinkers were empty stomach drinking, 6.07% drinkers were addicted drinking. ( 6 ) The drinkings had different increase on the male HBV carrier's total bilirubin and direct bilirubin, aspartate aminotransferase, alanine aminotransferase (P<0.05 ) . ( 7) The AUDIT questionnaires survey showed the prevalence of social drinking was 79.7% (745cases), hazardous and harmful drinking 18% (168cases) and alcohol dependence 2.4% (22cases). The mean of AUDIT total score and the constituent ratio of the hazardous harmful alcohol dependence (AUDIT 7~26 scores) reached peak at the age of 30 to 39 years old, being 4.05±5.04 and 27.8%, respectively. In the educational background, the primary school and below group had the highest scores and ratio, 4.21±5.21 and 27.9%, respectively, and the second is bachelor degree or master degree or doctor degree , it was 4.12±4.21 and 26.0%, respectively. Among the profession, the officer and the professional staff group had the highest score and ratio, they were 4.82±4.65 and 32.1%, respectively. According to the income, The cases whose monthly income was no less than 2000 yuan had the highest score and ratio, 4.26±5.43 and 27.2%, respectively. ( 8 ) The year drinking capacity and SAS scores were positively correlated with AUDIT scores in outpatient male HBV carriers (P =0.000, P =0.026). ( 9 ) The mean of SAS total score mean in alcohol dependence group was much higher than that in norm group (35.95±11.55 vs 29.78±0.46, P =0.020). The mean of SAS total score and BDI total score is significantly higher in alcohol dependence group's than that in moderate group and the dangerous group ( 35.95±11.55 and 10.45±8.95 vs 29.65±7.97 and 6.35±5.65 vs29.68±7.06 and 6.44±5.27) . ( 10 ) Multiple linear regression analysis showed that the influence factors of the outpatient male HBV carrier's AUDIT total score were sleep, the initial age of drinking, appetite in succession (P<0.05). Conclusion: ( 1 ) The month drinking rate and the year drinking rate of the outpatient's male HBV carrier were 35.0% and 58.2%, respectively, and the per capita year capacity for liquor of the drinker was 1.68±6.44L absolute ethanol. In addition, beer is the first choice. ( 2 ) Many questions were existed in the outpatient male HBV carriers such as heavy drinking, frequent drinking, fast drinking, high degree alcohol and mixed wine, only drinking and empty stomach drinking, underage drinking and addicted drinking, it cannot be ignored. ( 3 ) Drinking has influences on the outpatient male HBV carrier's liver function indexes (aspartate aminotransferase and alanine aminotransferase, total bilirubin, direct bilirubin), emotional state (anxiety and depression). ( 4 ) Among the male HBV carriers, those who were 20 to 39 years old, had bachelor degree or master degree or doctor degree, possess determinate specialized skill or economic income were high risk group and should be intervened specially.
