| Objective:Malnutrtion is frequently associated with esophageal can cer.The highly stressful response following esophagectomy may induce p ostoperative immunoSupp-ression.The aim of the present study was t o e valuate the Immunolodical Modulation effect and the nutritional status o f Supportan and Growth hormone in patients following esophageal canc er surgery.Methods:Forty-five patients who underwent the same surgical proc edure for thoracic esophageal cancer were randomized into three groups with 15 patients in each group:experimental group A received enteral nutrtion Supportan,experimental group B received entaral nutrtion Fresu bin with Growth hormone and C(control group)-PN group.Each group received iscaloric and isonitrogenous nutritiongal formulas on the same postoperative day which was started in the first 24 hours postoperative ly and continued for 7 consecutive days.The nutrition goal was 35~40 kcal/kg.day.A serious of nutrtional and immune parameters including se rum total proteins(TP),serum albumin(Alb),prealbumin(Pab),transferr-in(TF N),IgG,IgA,IgM,CD3,CD4,CD8,CD4/ CD8 were tested in one day bef ore operation and day 1,day 7 after operation.Results:There was no significant difference in the result of immu ne and nutritional parameter among the three groups before operation( P>0.05).On the pod1,all the serum potein parameters significantly decrea sed in all groups.serum IgG,IgA,IgM levels were decreased slightly.CD3, CD4 and CD4/CD8 significantly decreased postoperatively in all groups (P<0.05),There was no significantly changes of CD8.On the pod7,the c oncentration of serum total albumin,prealbumin,transferrin,serum IgG,Ig A levels,CD3,CD4,CD3/CD4 were significantly elevated in A and B gro ups.Compared with C group,A group was more significant(P<0.01).The 1 ength of hospital stay was shorter and no serious complication in entera 1 nutrtional groups.Conclusions:Early postoperative enteral immunonutrition of Suppor tan and rhGH for patients with esophageal cancer can significantly mod ulate the postoperative immun-suppressive responses,improve the nutritio nal status,decrease the occurrence of postoperative complication as well as the length of hospital stay. |
