Objective:To study marine collagen peptides'effect on glycometabolism, insulin secretion function and insulin sensibility of diabetic patient and investigate its effect on postoperative patient as clinical immunonutrition support.Methods:Part I: Clinical evaluation of marine collagen peptides'assistant hypoglycemic function1. Oral glucose tolerance test(OGTT):All the subjects go through an oral glucose tolerance test on the first day in-patient and one another on the 7th day after the administered with marine collagen peptide. The oral glucose tolerance test was handled like this: After 8-12 hours'fasting, patients of the test group(group A) were administered with marine collagen peptide(6.5g×bid, orally), while patients of the control group(group B) were administered with isopyknic volume's water. All patients intook 75g's glucose or 100g's steamed bread after 15-20 minutes, and then an examination of blood glucose level at 0h, 1h,2h and 3h would be taken in order to draw the glucose tolerance curve and calculate the area under the glucose tolerance curve. Compare the area under the glucose tolerance curves between the two groups. The fact that the area under the glucose tolerance curve in group B is significantly lower than that of group A(p<0.05) can draw to the conclusion that marine collagen peptide's function on improving glucose tolerance of diabetic is effective.2. Fasting blood-glucose lowering test: All patients went through an examination of fasting blood-glucose level at 7 AM. in every morning from the first day in hospital to the 7th day after intervention. The mean fasting blood-glucose level between two group was compared to each other. Significant differences of mean fasting blood-glucose level between the two group can draw us to the conclusion that marine collagen peptide's function of lowering fasting blood-glucose level of diabetic is effective.3. Influence of marine collagen peptide on insulin sensitivity index(ISI) and insuline secretion(IS):All subjects went through an examination of fasting blood-glucaose and fasting blood insulin level on both the first day in hospital and the 7th day after intervention. The ISI and IS of each subject was calculate based on the following formula: ISI = 1 / ( FBG×FINS); IS= FINS/FBG. Compared to the control group, significant higher enhancing rate of ISI and IS level of the test group can draw us to the conclusion that marine collagen peptide is effective on enhancing the level of ISI and IS of diabetic patients.4. Influence of marine collagen peptide on the period of time of blood-glucose adjustment-phase and incidence of night hypoglycemia: The mean period of time of blood-glucose adjustment-phase and incidence of night hypoglycemia of the two group were both observed and compared to each other in order to study the influence of marine collagen peptide on the period of time of blood-glucose adjustment-phase and incidence of night hypoglycemia.Part II: Evaluation of marine collagen peptide on postoperative patient as clinical immunonutrition support1. marine collagen peptide's effect on nutritional condition of postoperative patients: All subjects went through an examination including weight, upper-arm circumference, waistline, hipline and level of total protein, albumin, prealbumin, transferring on the first day in hospital, the 1st day after operation and the 7th day after intervention,in order to study the influences of marine collagen peptide on the changes of nutritional condition of diabetic patients.2. marine collagen peptide's effect on immunity condition of postoperative patients:All subjects went through an examination including C-reactive protein,IgG,IgM,IgA,and total lymphocyte count on the first day in hospital, the 1st day after operation and the 7th day after intervention,in order to study the influences of marine collagen peptide on the changes of immunity condition of diabetic patients.3. Comparison of average hospitalization days between the two group:Hospitalization days of all subjects were recorded in order to calculate the average hospitalization days of each group. And then the average hospitalization days of the test group would be compared to that of the control group in order to see if there is an significant difference between the two groups.Results:Part I: Clinical evaluation of marine collagen peptides'assistant hypoglycemic function1. It was demonstrated that the lowering rate of blood-glucose and the area under the glucose tolerance curve in group B was significantly lower than that of group A(p<0.05). marine collagen peptide's function on improving glucose tolerance of diabetic proved to be effective.2. In the Fasting blood-glucose lowering test, it was described that the changes of fasting blood-glucose of subject are related to the factor of grouping, which mean that the intervention of marine collagen peptide is related to the lowering of fasting of blood glucose. 3. As for ISI and IS, there were no significant change in group B. But in group B, both the ISI and IS have a significant elevation on 7th days compared with the 1st day ( p < 0.05).4. Patients in group B prove to have a significantly higher incidence of night hypoglycemia(p<0.05) and longer blood-glucose adjustmentphase than that of group A(p<0.05).Part II: Evaluation of marine collagen peptide on postoperative patient as clinical immunonutrition support1. It was demonstrated that all the detected nutritional factors of both groups were lowered after operation(p<0.05). Lowering rate of weight, upper-arm circumference and hipline of patients in test group was significantly less than that of control group, and the elevating rate of the level of albumin, prealbumin, transferring was significantly higher than that of the control group(p<0.05).2. The elevating rate of the level of blood IgG, IgM and total lymphocyte count was significantly higher than that of the control group(p<0.05).3. The average hospitalization days of the test group proves to be significantly shorter than that of the control group(t=-2.388,P=0.019<0.05).4. The blood C reactive protein level of the test group was significantly lower than that of the control group.Comclusions:1. Marine collagen peptides proved to be effective in enhancing blood glucose tolerance of diabetic patients and lowering the fasting blood glucose of diabetic patients, which mean that its function of Assistant Hypoglycemia is effective.2. Marine collagen peptides were effective on enhancing the level of ISI and IS of diabetic patients.3. Marine collagen peptides can shorten the blood-glucose adjustment-phase of diabetic patients 4. Marine collagen peptides can lower the incidence of night hypoglycemia of in-patient diabetic patients.5. Marine collagen peptides are effective on improving the nutritional condition of postoperative patients.6. Marine collagen peptides are effective on improving the immunity condition of postoperative patients.7. The use of marine collagen peptide to postoperative patients can shorten their average hospitalization days.8. Marine collagen peptides may have certain effect on anti-inflammatory capability of postoperative patients.
