| Objective To investigate the manifestation of CT perfusion imaging and to evaluate the diagnostic value of CT perfusion imaging technique in patients with ischemic cerebrovascular disease.Methods Digital substraction angiography(DSA)and CT perfusion imaging(CTP)were performed in 32 patients suspected with ischemic cerebrovascular disease.The different vasculopathy have been found in all cases, then all cases were divided into different groups according to the state of stenosis.The cerebral perfusion parameters such as cerebral blood flow (CBF),cerebral blood volume(CBV),time to peak(TTP)have been caculated after CTP imaging.The patients' result of plain CT scan,CTP and DSA have been analyzed.Result①32 cases have been divided into 6 group according to DSA. There are 11 cases with the stenosis of unilateral MCA,13 cases with the stenosis of unilateral ICA,1 case with the stenosis of bilateral ICA,3 cases with the stenosis of unilateral vertebral artety,2 cases with the stenosis of basilar artery,and 2 cases with the generally arteriosclerosis.②The difference of detection rate between plain CT scan and CTP imaging have been found in 32 patients,(p<0.001)the detection rate of CTP imaging is higher than plain CT scan.③The detection rate of cerebral perfusion parameters are different,TTP is more sensitive.The measure value of CBF and CBV is no different between normal side and affected side in cerebral hemispheres(p>0.05),but that of TTP is different(p<0.001).The measure value of CBF and CBV is no different between normal side and affected side in middle cerebral artery(MCA) blood-supply area in 11 patients with the stenosis of unilateral MCA(p>0.05), but that of TTP is different(p<0.001).And the measure value of CBF and CBV is no different between normal side and affected side in internal carotid artery (ICA)blood-supply area in 13 patients with the stenosis of unilateral ICA(p>0.05),but that of TTP is different(p<0.05).④The abnormal blood perfusion (TTP)area are the same in some patients with the stenosis of ICA and MCA, the same result also be found in the patients with the stenosis of vertebral artety and basilar artery.⑤There is no dependability between the rate of stenosis and the prolongation of TTP in 11 patients with the stenosis of unilateral MCA(p>0.05),and also no dependability between the rate of stenosis and the prolongation of TTP in 13 patients with the stenosis of unilateral ICA(p>0.05).Conclusion The detection rate of CTP imaging is higher than plain CT scan,so that the responsibility vessel can be detected more earlier,and severe consequence can be avoided.The TTP is the best sensitive index among that 3 cerebral perfusion parameters.The abnormality of CTP can hint the stenosis of responsibility vessels and guide the advanced examination.The is no dependability between the rate of stenosis and the prolongation of TTP. |
