Objective To explore the influencing factors of therapeutic effect in 131I treatment of Graves disease.Methods The 172 patients with Graves disease underwent 131I treatment who were followed up for 1 year.Patients were divided into 3 groups on therapeutic effect cured group,not-cured group and hypothyroidism group.We assess the relationship between age,gender,thyroid texture,disease course, whether taken ATD before treatment,the highest thyroid uptake ratio of 131I, thyroid weight,the visual index of thyroid uptake(radioactive counts in thyroid area/radioactive counts in neck area,sparse location radioactive counts in thyroid area/radioactive counts in thyroid area),thyroglobulin antibody,thyroid mitochondria antibody with effect by SPSS software.We made diagnostic cut-off value of sparse location radioactive counts in thyroid area/radioactive counts in thyroid area by receiver operating charateristic curve(ROC).We did single factor analysis for each influencing factors,Logistic test was used to analyze variable parameters before treatment.Results After one year 85.5%of the cases were cured,14.5%were notcured, and 16.9%become hypothyroidis.The result of ROC curve analysis showed the best diagnostic cut-off value of sparse location radioactive counts in thyroid area/radioactive counts in thyroid area is 0.916,and the responding sensitivity and specificity are 0.861 and 0.652,respectively.Discrepancy of the cure rate,not-cured rate and hypothyroidis rate were different in thyroid weight, thyroid texture,the highest thyroid uptake ratio of 131I,the visual index of thyroid uptake,thyroglobulin antibody and thyroid mitochondria antibody(P<0.05) The highest thyroid uptake ratio of 131I,thyroid weight,radioactive counts in thyroid area/radioactive counts in neck area were different in 3 groups(P<0.05).The difference of sparse location radioactive counts in thyroid area/radioactive counts in thyroid area,thyroglobulin antibody,thyroid mitochondria antibody were statistical significantly in cured group and hypothyroidism group,the similar results were also found in not-cured group and hypothyroidism group(P<0.05)Logistic stepwise regression analysis revealed that the highest thyroid uptake ratio of 131I,thyroid weight,the visual index of thyroid uptake,thyroglobulin antibody,thyroid mitochondria antibody was significant for the effectionConclusions 131I is a better treatment for Graves disease.When we determine therapeutic dose we could consider some factors which are of significant,such as the highest thyroid uptake ratio of 131I,thyroid weight,the visual index of thyroid uptake,thyroglobulin antibody,thyroid mitochondria antibody there will be some clinical significance for the patients before treatment.In order to improve the effection of 131I on Graves disease and reduce the incidence of hypothyroidism.
