Objective:To investigate the clinicopathological features of minute gastric cancer(M-EGC)and small gastric cancer(S-EGC),help to provide a basis for selecting rational therapy.Methods:31 patients with M-EGC or S-EGC were admitted to the second affiliat ed hospital of Zhejiang University between 2004 and 2007.Their clinicopathological data were analyzed retrospectively.Results:11 patients with M-EGC and 20 pantients with S-EGC were diagnosed by gastroscopy with biopsy.10.6%(31/283)had M-EGC and S-EGC in early gastric cancer.The differentiated cancer in M-EGC was 72.7%,much higher than 55%in S-EGC group.The intramucosal cancer in M-EGC group was 81.8%,much higher than 40%in S-EGC group,which was statistically significant(P=0.029,<0.05).None of S-EGC patients had lympy node metastasis,however,1 of 31 M-EGC patients had was lympy node metastasis.Both groups had not blood vessel and lymphatic vessel invasion.The differentiation concordance rate between superficial lesions and invasive fronts in M-EGC group was 81.8%,much higher than 40%in S-EGC group.In total, 26%of the cancer revealed lower differentiation degree in invasive fronts than in superficial lesions;3.2%of the cancer revealed higher differentiation degree.Conclusion:M- EGC and S-EGC are 10.2%of early gastric cancer,gastroscopy with biopsy was the main diagnostic measure.S-EGC has lower histopathological differentiation,deeper depth of tumor invasion,higher rate of lympy node metastasis than M-EGC.With the progress of tumor,both M-EGC and S-EGC have the variation of histopathological type.The most cases of M-EGC and more than half of S-EGC(The differentiated,without lympy node metastatic,without blood vessel and lymphatic vessel invasion patients)are the favorable cases for Endoscopic therapy.
