Objectives To comparing enteral nutrition(EN) and parenteral nutrition(PN) in the treatment of severe acute pancreatitis(SAP), in order to elucidate the feasibility and advantage of EN. Method Fifty-two cases of SAP were retrospectively analyzed and divided into 2 groups. There were 30 cases in enteral nutrition group and 22 cases in conventional therapy group. The general conditions were analyzed. The nutritional status and biochemical was determined the 7th and 14th day after giving the nutrition support,and the incidence of the local and systemic complications, mortality rate, cure rate, hospital stay day and hospital expenditure were compared. Result The blood glucose, and the levels of serum and urine amylases were lowered significantly and the serum calcium were increased obviously in both PN and EN groups two weeks after giving the nutrition support (P<0.05). In contrast with PN, the serum albumin were obviously high and white blood count were lowered In EN group (P<0.05). The rate of serious complications such as ARDS and shock, hepatic function injury, mortality rate and hospital fees in EN group were significantly lower compared with PN group (P<0.05). Durations of restoring oral in-take and hospitalization in EN group were obviously shorter than those in PN- group (P <0.05). Conclusion As the major treatment of SAP, it is safe and effective to give early EN support to patients with no contraindication. It was shown that the nutritional status of the patients was markedly improved, complication were reduced, mortality rate were lowered, hospital stay days were shortened and expenditure in the hospital was cut down after EN support.
