Bispectral Index Applied To Sedation For Patients In Paediatrics Intensive Care Unit

OBJECTICV: Discussing the applicability of bispectral index(BIS) used in monitoring sedatives in paediatrics intensive care unit.METHODS: prospective study.42 patients in PICU of our hospital, which were needed to be sedated for the illness and rescuing, were given Midazolam by continuously intravenous injection. Keeping the patients calm and the Score of Ramsay between 3 and 5 point were suitable extent.Recording the sedative depth according to the Score of Ramsay , simultaneously recording the value of BIS .Analyzing the relationship between Ramsay score and BIS value and the factors influenced BIS. RESULTS: 42 critically i11 children in PICU of Shanghai Jiaotong University affiliated children hospital from June 2007 to March 2008。Including 13 cases that age was form 6 mon to 1 year.24 boy cases (57.14%),18 girl cases(42.86%),the mean age was 21.23±5.56 mon(6mon-13yrs).The objects should remove these cases: Cerebral palsy,cerebral developmental retardation,mental retardation,epilepsy,scalp edema,Intracal masses,haematoma etc. After applying Midazolam patients began to produce a marked effect and BIS began to change 3-5minutes. so when the Ramsay score were 2,3,4,5,6, the corresponding value of BIS were 84.66±4.68,79.89±5.27,64.65±5.13,55.80±4.97,49.72±6.00;according to Spearman's scale analysis, the correlation coefficient of BIS and Ramsay score is -0.8879(P<0.01),which has evidently statistical significance. The correlation coefficient of little children team (age from 6mon to 1year) was-0.8427,no evidently statistical significance(P>0.05).CONCLUSION: BIS is an objective method on sedative monitoring for PICU patients. Better exercisables and easily. BIS was a proper range for sedative depth when the BIS value is in range 60-80.BIS used in sedative monitoring had complementarily with Ramsay score.
