BackgroundClinical attachment phase is the critical stage to develop clinical practice skills and thinking ability.The quality of teaching in hospitals and clinical practice performance of students arc extremely important to train qualified doctors.However,the development and transformation of society bring great impact on the clinical teaching. How to improve the quality of clinical teaching and performance of clinical practice have become the urgent problems that medical educators need to confront and solve.PurposeTo clear significant impacting factors on the quality of clinical teaching and improve the quality of clinical teaching,we studied students' performance of the Objective Structured Clinical Examination and the effect of factors in clinical practice.Materials and MethodsFive-year medical students(n=117)in China Medical University were divided into 8 groups according to their internship hospitals in 2007.We collected their performance of graduate examination and internship information of eight clinical teaching hospitals by surveys.The students' scores were aggregated and analyzed by SPSS13.0.ANOVA and SNK-q test were used to compare student performance differences among hospitals. K-W test was used to compare the differences of impacting factors among the various hospitals.Linear correlation analysis was used to observe the relationship between the factors and students' performance.ResultsStudents' total scores and OSCE scores were significant difference among the hospitals by ANOVA;P<0.05 and P<0.01.The scores of written examination was not significant difference by ANOVA;P>0.05.The OSCE scores of students in Hospital A and C were lower than students' in other hospitals,and the OSCE scores of students in Hospital B and G were higher than students' in other hospitals.The scores of students in Hospital G and B were higher than students' in Hospital A and C by SNK-q test.There were differences in the provision of operational opportunities for students and the teaching management and the level of teaching hardware among hospitals by K-W test.There were not differences in the number of beds in charge of students and the teaching attitude of teachers and the emphasis on teaching among hospitals by K-W test.There was a positive correlation between the level of teaching hardware and students' OSCE performance among hospitals.Discuss and ConclusionsThere exist differences in the teaching conditions and learning environment of the hospitals,and students' internship performances vary widely.So the students' OSCE performances are differences among hospitals.The provision of operational opportunities for students is an important factor that affects their clinical skills.Because students fewer contact with patients directly and their clinical operation are greatly affected,how to increase clinical practice opportunity has become an important problem.The teaching management may work with the medical and other work their mutual coordination and promotion. Well-functioning clinical teaching management system would create a good learning environment,in which students could learn clinical skills and train clinical thinking ability.To improve the level of teaching hardware would create the overall optimization of the teaching environment and conditions.The results show that students' OSCE performance advanced with the level of the hardware enhancing,thus to increase the hardware level of the teaching hospitals play an important role.However the investment of teaching is huge and slow and many teaching hospitals are relatively backward.To strengthen teaching facilities hardware input has become a top priority for them.The number of beds in charge of students and the teaching attitude of teachers and the emphasis on teaching are important impacting factors.In charge of a certain number of beds in clinical practice is one of the main requirements in internship.The responsibility and enthusiasm of teachers are important guarantees for clinical teaching. And the work of teaching is the driving force to improve the level of health care.There is a positive correlation between the level of teaching hardware and students' OSCE performance among hospitals.Therefore hospitals should focus on strengthening the teaching of inputs and improve the hardware level.
