Relative Factors Analysis On The Prognosis Of Patients After Severe Head Injury

Objective:To analy relative factors on the prognosis of patients after severe head injury and provide the scientific clinic basement for elevating therapia level of it .Methods: 708 patients with severe head injury from January,1996 to December, 2005 in the first affiliated hospital of Nan Chang University were included in the study. We list body temperature﹑GLU﹑Na﹑CT cisterma ambiens﹑sexual﹑the count of WBC﹑systolic pressure﹑age﹑the time frome being injuried to admission﹑history of past illness﹑complication,associated injury﹑GCS score﹑herniation of brain as clinic surverying index .when patients discharged , We emloy GOS class standard to score the patients about their prognosis,patients withⅠ～Ⅱgrade are regarded as unfavourable prognosis and withⅢ～Ⅴgrade as eusemia. we study them by the way of simple factor analysis and multiple factors Logistic Regress Analysis by Spass14.0 software.Results:275 patients ( 38.84% ) are dead among 708 STBI patients.356 patients(50.28%) are unfavourable prognosis,and 352 patients(49.72%)are eusemia. we find that: GLU﹑Na﹑complication﹑herniation of brain﹑CT cisterma ambiens﹑age﹑ the admission body temperat﹑GCS score﹑systolic pressure, by turns, have more significant differences than others factors effecting the prognosis of patients with severe head injury. Although the count of WBC﹑sexual﹑history of past illness﹑associated injury have significant differences only in simple factor analysis in differe prognosis subgroud(p<0.05), the time frome being injuried to admission have not differences.Conclusion:GLU﹑Na﹑complication﹑herniation of brain﹑CT cisterma ambiens﹑age﹑the admission body temperat﹑GCS score﹑ systolic pressure have more significant differences amony the factors influencinging the prognosis of patients with severe head injury.To aim directly at above influencing factors,promptly taking the scientific means of prevention and cure is the key point of lowering the death rate , mutilation rat and improving the prognosis of patients with severe head injury.
